Objective:We observed the levels of serum immunoglobulin G(IgG) and thedynamic changes in patients with acute cerebral infarction (ACI)of before berberinetreated and after, to investigate IgG and mononuclear cells ratio(MONO%) of peripheralblood and the relationship between them and the neurologic deficits in the Acute phaseof infarction, further study of its clinical and pathologic physiological significance, toexplore the cerebral protective effect of berberine and its probable mechanism ofpathophysiology.Methods:We choosed the healthy check-up of43cases as control group,42patients with acute cerebral infarction as cerebral infarction group,in accord withstandard of gender and exclusion,age composition of comparable(P>0.05).Then wedivided the patients with acute cerebral infarction into common group and berberinegroup according the different treatment,and in accord with standard of gender,agecompositon and NIHSS scores of comparable (P>0.05). All the patients adopted theway of randomized controlled trials. By the rate nephelometry method respectively inthe groups,serum IgG, and by the Japanese sysmex XE-5000automatic analyzer and thesupporting reagent, MONO%of peripheral blood,observe the change of serum IgGlevels within48hours of onset and the first10days after admission.For cerebralinfarction patients neurologic deficits degree by the national institutes of health strokescale(NIHSS) scores were evaluated in pathogenesis,within48hours of onset and thefirst10days to assess twice. All the results on baseline were marked as “1” and thoseon the10thday were marked as “2”, and (1subtract2) was marked as D-value.All datausing SPSS11.5statistical software, test of significant level for bilateral inspectionP<0.05. Results:1.The serum IgG1level of cerebral infarction group in common group andberberine group were both significantly higher than control group,but have nosignificant differences. The serum IgG2level of cerebral infarction group in commongroup were significantly higher than control group(P<0.05), and which weresignificantly lower than control group,but have no significant differences in berberinegroup.2. The serum IgG levels of two times with the first and the second times MONO%have significant positive correlations(p<0.05,P<0.01).3.The serum MONO%1of cerebral infarction group have no significantcorrelations with two times NIHSS scores.The serum MONO%2of cerebral infarctiongroup with NIHSS2have significant positive correlations,but have no significantcorrelation with the NIHSS1scores.The correlation coefficient of the serum IgG level two times with the first and thesecond times NIHSS in cerebral infarction group were both positive number,that is theyhave positive correlation trend,of which the correlation coefficient of IgG2withNIHSS2was0.283(P=0.07),have approximate positive correlations.4.The level of serum IgG1of berberine groups was no significant differencecompared with the common groups,but the level of serum IgG2of berberine groups wassignificant lower than those of common groups(P<0.05).The level of serum IgG2of berberine groups compared with IgG1havesignificantly reduced(P<0.05). the serum IgG2level of common groups compared withIgG1have significantly increased,but have no significant difference.The serum IgG D-value level of berberine groups with common groups were havesignificant differences(P<0.05).Conclusions:1.The serum IgG level of cerebral infarction groups increased significantly,indicate that abnormal humoral immune may participate in in the acute phase ofcerebral infarction.2.The serum IgG level of acute cerebral infarction patients have significantlypositive correlation with the serum MONO%level,indicate that the serum of IgG andthe serum of MONO%may have closely correlation in the process of thepathophysiology of cerebral infarction.3.The serum MONO%was highly positive correlative with NIHSS scores in the patients of acute cerebral infarction, which indicated that the level of MONO%mayreflect severity of the neurologic deficits,and the serum IgG level of cerebral infarctiongroup also may approximate reflect severity of the neurologic deficits,but the exactlyclinical significance need to further research by increasing sample number.4.Berberine can decreased the level of serum IgG, indicate that berberine mayrelieve the abnormal humoral immune reaction in the acute phase of cerebralinfarction.
