Clinical Effects Of Gasless Laparoscopic Operation For Ectopic Pregnancy

Objective:To investigate gasless laparoscopic operation in the surgical treatment for ectopic pregnancy. Methods:In Shanghai Fengcheng Hospital from Jan.2007to Dec.2011, prospectively studied were60cases of ectopic pregnancy, which were randomly divided into2groups: Gasless Group (30cases) was given laparoscopic operation with gasless laparoscopy, and Pneumoperitoneum Group (30cases) with CO2pneumoperitoneal laparoscopy. The operating duration, blood loss, surgical complications, postoperative intestinal function recovery, postoperative hospitalization time, postoperative morbidity, and hospitalization expenses were compared between the two groups. Results: The operating duration, intestinal function recovery time, hospitalization expenses, Indexes of hemodynamics after operation, the pain of shoulder, nausea, and vomiting were significantly different between two groups (P<0.05). The amount of bleeding, postoperative hospital stay were not significantly different between the two groups (p>0.05), and the preoperative DBP, SBP, SpO2%, MAP, and HR were also not significantly different between the two groups (p>0.05). The operations of all tthe patients of the two groups were successful. In all cases there were not abdominal, vascular, abdominal visceral injury, or subcutaneous hematoma and hemorrhage, or other complications such as hypercapnia, acidosis, subcutaneous emphysema and CO2gas embolism. Conclusions:Gasless laparoscopic operation, without CO2pneumoperitoneum and the consequent complications, is both safe and minimally invasive, and the effects on hemodynamics is small, and the postoperative intestine recovery was as fast as that in laparoscopic operation with CO2pneumoperitoneum. Therefore Gasless laparoscopic operation should be clinically as useful as CO2-pneumoperitoneum-laparoscopic-operation in the surgical treatment for ectopic pregnancy.
