Objective:To study applications, advantages and feasibility of the exemption gasless laparoscopic surgery (LC). Methods:30 cases which were underwent cholecystectomy resection with gasless LC for gasless group,30 cases which were underwent cholecystectomy with gas LC for gas group, and these cases were collected from our hospital from March 2009 to June 2010. Average operative time, median intraoperatie bleeding, postoperative bowel function recovery time, length of hospital stay, preoperative and postoperative blood gas analysis, the postoperative complications, postoperative hospitalization cost were analysis and compared in two groups. Results: The surgeries were successfully completed in two groups. There were no complications such as abdominal wall, blood vessels, abdominal viscera damage, no subclavial hematoma and haemorrhage, any hypercapnia, acidosis, subcutaneous emphysema and pneumomediastinum or pneumothorax, CO2 gas embolism. There were no significant difference in operative bleeding, average operation time, average time of bowel function recovery, average length of hospital stay, average hospital expenses, the postoperative complications in two groups (P>0.05). There were no statically differences in preoperative paO2, pCO2 and DBP, petCO2, spO2%, PH value of hemodynamic analysis in two groups (P>0.05).There were significant differences in preoperative SBP, MAP of two groups (P<0.05). Postoperative pCO2, spO2%, PH value weren't statistically different in two groups (P>0.05). Postoperative DBP, SBP, MAP, and paCO2, petCO2% were significantly different in two groups (P<0.05). Conclusion:Gasless laparoscopic surgery (LC) is a safety surgery with minimally invasive, quick postoperative recovery and there are no complications of CO2 gas laparoscopic surgery. Gasless laparoscopic surgery (LC) has minimal effect to human body hemodynamic, and it has a valuable aspect in clinical applications.
