| OBJECTIVE1. To discuss the disinfection effect of Electrolyzed oxidizing water, Glutaraldehyde andOrtho-phthaldialdehyde to the bacteria of the digestive endoscopies in order toprovide a choice to the best disinfection method.2. To explore the HBsAg positive rate on the surface and cavity of the gastroscope thathad been used by hospitalized patients who had infected HBV.3. To confirm the cleaning effect to gastroscope had been used the manual cleaningworks and disinfection effect of different disinfectants to the HBV.METHODS1. A total of210gastroscopes and enteroscopes were selected by a randomly ways.According strict cleaning process of the manual cleaning method, so as to comparisonthe amount of bacteria in the digestive endoscopies before and after cleaningapproaches, and then to find the disinfection effect of different kinds of thedisinfectants. 2. A total of120continuous inpatients were randomly selected in a hospital from Jun toDec in2012. HBsAg positive rate were compared between outside and lumen of thegastroscope that were used ELISA, Using chi-square test to compares the differentHBsAg positive rate.3. Through simulation experiment, Using the high concentration of serum hepatitis Bvirus serum to pollute the appearance and inner cavity of gastroscope. Adoptingfluorescence quantitative PCR detection to detect the hepatitis b virus content of thesamples that obtained at before and after cleaning and disinfection, Using the paired ttest and t test method to contrast different cleaning effects and kill rate of thedisinfectants.RESULTS1. Inner cavity flushing fluid of the digestive endoscopies was used bacteria culture andcolony counting. To compare different effects on the three disinfectants were appliedin the process of cleaning. After the three disinfectants were sterilized in prescribedprocedures, there were no viable bacteria and the bacteriological clearance rate was100%in the entire gastroscopies, however, but the three different disinfectantssterilized efficiency of the colonscopies were different, the Electrolyzed oxidizingwater was44.44%〠Glutaraldehyde was54.84%and Ortho-phthaldialdehydewas75.00%, Three different disinfectants sterilized effect to colonscopies arestatistically significant(P<0.05).2. The HBsAg positive rate of the gastroscope in the appearance was36.37%,but in innercavity was58.33%, there is statistics significance(P<0.05)difference between the twoparts according the chi-square test.3. The average HBV-DNA between outside and lumen of the gastroscope are statisticallysignificant(P<0.05)after the manual cleaning works.4. The killing rate of hepatitis b virus on the gastroscope after use the electrolyzedoxidizing water, glutaraldehyde and ortho-phthaldialdehyde on gastroscope were91.26%,92.96%and86.76%respectively, however, the cavity of the gastroscope had been disinfected were0IU/ml.CONCLUSIONS1. Different disinfectants could be selected in different endoscopies. The best sterilizedeffect of gastroscopies is Electrolyzed oxidizing water and2min is the best choose;similarly, the best sterilized effect of colonscopies is Ortho-phthaldialdehyde, it needs5min.2. The HBsAg positive rate on the cavity of the gastroscope was greater than theappearance36.37%.We should attach importance to the cleaning and disinfectionmeasures of the endoscopies that had been used on the outpatients, hospitalizedpatients and HBeAg positive patients.3. The effect of manual cleansing and disinfection on the cavity were better than theoutside.4. The results of the disinfection effect to gastroscope shows that lumen disinfection isbetter than the appearance, and the best sterilized effect of HBV is Ortho-phthaldialdehyde. |
