Objective: The present study was designed to investigate the relationshipbetween subclinical hypothyroidism influence on blood coagulation system andthe severity of coronary heart disease.Methods: A total of the patients (n=285) with coronary artery disease weredivided into2main groups,110with single vessel lesion and175with multiplevessel lesions. The patients are collected from general hospital of ningxiamedical university from Oct.2010to Oct2012in Cardiovascular internal medicinedepartment. The two groups were retrospective cross-sectional studied, accodingof whether or not TSH is greater than7.0uIU/ml.Records of the clinical data,bodyweight index,systolic blood pressure and diastolic blood pressure,detection offreeT3,freeT4,T3,T4,thyroidstimulating hormone,total cholesterol,triglyeeride,high density lipoprotein cholesterol,low density lipoprotein cholesterol,fasting blood-glucose, glycosylated hemoglobin,blood uric acid,mean plateletvolume,fibrinogen,hemoglobin and coronary artery lesion counts.Furtheranalysis of TSH and the index of correlation.Result:1、The age,sex of single vessel lesion and multiple vessel lesions showedno significant difference(P>0.05);levels of BMI in multiple vessel lesionsgroup were higher than single vessel lesion group (P<0.05);2、Levels of TSH in multiple vessel lesions group were higher than single vessel lesion group (P<0.05);There was no significant difference of T3、T4、FT3、FT4between the two groups(P>0.05);3、Group of multiple-vessel lesions had much more subclinical hypothyroidismproportion than single vessel lesion group;however,there was no significantdifference of diabetes and hypertension proportion between the two groups(P>0.05);4、Group of multiple-vessel lesions had higher serum levels of TC,TG,LDL-C,MPV,FIB,while had lower serum levels of HDL-C(P<0.05)than single vessellesion group;however, there was no significant difference of FPG、HbA1c、SBP、DBP、UA、HGB between the two groups(P>0.05);5、In multiple-vessel lesions group,patients with subclinical hypothyro-idism had higher levels of BMI、TC、TG、LDL-C、MPV、FIB,and lower levels ofHDL-C、HGB than those with euthyroidism(P<0.05),there was no significantdifference in FPG、HbA1c、SBP、DBP between the two groups(P>0.05);the singlevessel lesion group has the same result;6、Compared multiple-vessel lesions group with euthyroidism and singlevessel lesion group with euthyroidism,levels of TC、TG、LDL-C、MPV、FIB inmultiple-vessel lesions group with euthyroidism were higher than single vessellesion group with euthyroidism (P<0.05);levels of HDL-C was lower;there wasno significant difference in HGB between two groups(P>0.05);7、Spearman linear analysis showed that TSH were positively related withBMI(R=0.350; P=0.000)、TG(R=0.307;P=0.000)、TC(R=0.204; P=0.001)、LDL-C(R=0.225;P=0.000)、 MPV(R=0.323; P=0.008)、 FIB (R=0.407;P=0.008).The correlationcoefficients were much higher than those of other parameters;while TSH wereno postively related with HDL-C(P>0.05).Conclusions: Level of MPV and FIB are increased in patients with subclinical hypoth-yroidism,and these are related to the coronary disease severity. Therefore,in clinical patients with subclinical hypothyroidism shoud routine examinationof MPV and FIB,can regard as a risk factor of coronary artery disease.
