| Research backgroundHemorrhoids is human-specified disease. According to investigation, the incidence of anal and rectal disease reaches to59.1%. Due to the wide existence of hemorrhoids, the therapeutic method towards hemorrhoids has developed very quickly. External dissection and internal ligation operation, the most traditional and one of the most important operation form currently, is a widely used and classic operation form. Therefore, postoperative pain has become the main misgiving for most mixed hemorrhoid patients. For some patients, the postoperative pain is the most tortured pain during all their life span. The research shows that early period pain after operation is the main pain for the patients. The postoperative pain can not only cause the corresponding syndrome, but also cause series of pathophysiology changes, or even affect the operation quality and the life quality for the patients in the recovery period. In modern medicine, though oral pain medications, local injection of drug and pain medication infusion pumps can reduce the patients’pain, the followed side effects generate such as urine retention, delayed wound healing, pain killers’ stimulation to the Gastrointestinal mucosa and so on. Most therapeutic methods, including daily fumigation of anal local parts, medicine changes laser therapy, electric message and wound treatment, demand to patients to expose the wound part, which is unfavourable to patients’ rehabilitation from the clinical rehabitation point of view. The past acupuncture anaigesia stimulates the wounded part and demands the patients to keep supine nd prone positions, which equals to the second stimulation to the patients. According to traditional Chinese medicine acupuncture theory, Erbai acupoint belongs to special therapeutic functions. It has no side effects to wounded parts after operation. And it proves that Erbai acupoints acupuncture have positive role to reduce the postoperative pain.PurportThe thesis aims to research the assessments of curative effects of different methods towards postoperative pain for fixed hemorrhoids, including filiform needle acupuncture, acusector, acupoints application and so on.The topic take the external dissection and internal ligation patients in Liaoning Anal and Rectal Hospital as the research objects, through the comparison the intervention methods such as filiform needle towards Erbai acupoints, acusector and acupoint application with the common methods of oral pain medications such as Diclofenac potassium capsule towards the post operative pain, adopting clinical evaluation standard, from the clinic syndrome point of view, to research the pain medication condition after anal and rectal operation, explore the corresponding advantages of different invention methods about Erbai acupoints, find out the most effective and efficient operation method and provide the clinical evidence for Anal disease clinical diagnosis and treatment methods and normative operations in it and for subject of anorectal diseases in Chinese medicine treatment plan optimization, and clinical application of capacity-building "from the Provincial Health Department on the postoperative analgesic.Materials and MethodsOn the basis of random, comparison and blind principles and diagnostic and inclusion criteria, during June2011and March2012,160patients, consist of84male patients and76female patients, are involved in this research. The patients are divided into4groups, with40per group. The therapeutic group mainly focuses on the anal postoperative pain condition with the different methods, such as filiform needle, acusector, acupoint application and moxibustion about Erbai acupoints. The comparison group emphasizes on evaluation of the postoperative pain and get statistics analysis by using VAS. Meanwhile, compared pain time, the nature of the pain, the average length of stay, thepostoperative two circumstances, postoperative anal bulge and edema, safety and adverse reactions and other indicators, and then evaluate the curative effect of stimuation methods about Erbai acupoints and oral Diclofenac potassium capsule to cure the postoperative pain.ResultAmong160external dissection and internal ligation patients, the research shows there are no evident differences between two groups on age, gender, wounded area and so on. The four group patients’ materials are comparable after the chi-square test and T test (P.>0.05)1、The four groups are evaluated through VAS4,6,12,24,48,72hours after operation. The research shows that all four group patients evaluation are all improved, compare with the condition before the operation, the differences has statistics meaning. The research also shows that the different methods with Erbai acupoints can effectively cure the postoperative pain after external dissection and internal ligation. Through clinical research to determine that Erbai acupoints is an effective way to cure the postoperative pain.Conclusion1、 Acusector, filiform needle and acupoint application, to some extent, can reduce or even stop the osoperative pain and receive better effects.2、Erbai acupuncture has the Character of simple operation, safety, low-price andsimple application, which is the easiest and most effective pain medication method after mixed hemorrhoids operation.3、 Acusector intervention with Erbai acupoints has better effects to reduce the pain after external dissection and internal ligation. This shows that acusector is the most effective method to cure the postoperative pain after the operation and the acupoint application is the most efficient method to kill the pain after the operation.4. Acupoint application is not the treatment of traumatic, and therefore provides a new option for clinical. The combined application of a variety of treatment methodsmay be further applied, and provide the basis for the clinical pathway. |
