New Reform Research Situation Of Public Hospital Compensation Mechanism Reform

Reform on Public hospitals has become a hot and difficult issue from2009when a new round of medical and health system reform had begun. While the reform of the compensation mechanism is the key to the success of the reform on public hospitals. The compensation channels mainly include financial assistance, medical services fees and income from pharmaceuticals three parts in public hospital. With the state financial input in health recently decreasing, while the medical services fees is the limit below-cost pricing policy, public hospitals in maintaining the daily operation, improve service levels and expansion of the scale had to rely on drugs addition income to compensate, leading to reduce the public welfare tendency of public hospitals, medical treatment is expensive and difficult. Especially charges for drug has become more and more expensive. The social issues directly affects the whole society’s harmony and stability.This paper apply public management, health economics, public policy studies, hospital management theory, using literature analysis method, analysis of a typical case, comparison of methods, qualitative analysis and quantitative analysis method. On the basic of introducing the connotation and denotation of conception, the public supply theory, market and government malfunction, stakeholder theory and the philosophy of incentive compatibility were used in analysis in order to mouse out the balance of equity and efficiency of medical service. This paper opened out that the drug price addition policy has led to the rise in drug costs and "drugs to support medical" phenomenon, and described the influences caused by the policy and appraised the advantage and disadvantage of the policy from the angle of hospital long-term development. In the same time, this paper looked back the back-ground of the reform of the public hospitals compensation mechanism, and analyzed the problems and its reasons of the current compensation mechanism for public hospitals. The analysis is also combined with domestic and abroad experience and indication in education, provide the experience for public hospital compensation mechanism of Chinese medical health system reform."On Deepening the views of medical and health system reform" proposed,"to gradually reform or abolish drugs price addition policy". The essay conduct a comprehensive study for the issue of compensation mechanism in the public hospitals, and proposed to abolish the policy of drug-plus compensation mechanism in the reform of public hospitals measures and suggestions which mainly include seven aspects. Firstly, take the construction of practical and sharing medical health information system; secondly, the compensation can be improved through health resources distributing and the design of the Public Hospital Performance Evaluation Index System consistent with the nature of the public welfare, thirdly, continuously improve the medical service price system; fourthly, pharmaceutical additional service charges, don’t plus drugs to make up after the abolition of the income gap; fifthly, improve the medical insurance service system to promote the third-party payers; sixthly, further consummate drug production and circulation system; seventhly, carry out cost accounting, and the fund of the public hospitals’ compensated system should be allocated and be supervised effectively. In order to people’s problem difficulty and expensive with seeing a doctor, this study analyzes various kinds of compensation measures and strategies, and proposes specific integrated scheme, which requires us to set up the philosophy of incentive compatibility, only in this way can inspire all stakeholders’initiative, so as to stick up for people’s health in good condition. Thus, I hope this thesis would has some important realistic meaning and value to promote the operation of medical and healthcare system reform effectively.
