Anemia In Patients With Lung Cancer Investigation And The Analysis Of Related Factors

Objective:Anemia is one of the complications commonly occurred during the process of tumors, while among all the tumors, the incidence of Anemia in lung cancer is relatively high, not only seriously affecting the quality of live for patients, but influencing the treatment and prognosis for tumors as well. In our study, we adopted the retrospective method to analyze the related clinical factors for Anemia from the aspects of demography, diseases, therapeutics and syndrome of Traditional Chinese Medicine (TCM), by investigating the occurrence status and distribution characteristics for Anemia in patients with Lung Cancer, in order to provide the evidence for establishing the targeted Interventional program of integrated Western medicine and TCM.Methods:We collected the data about the demography, diseases, therapeutics and syndrome of TCM from primary lung cancer patients, who were admitted in our hospital for the first time from January2010to December2011, entered all the information, established the database and adopted the SPSS19.0software for statistical analysis. The descriptive analysis of frequency and rate was used for all the variables. The Chi-square test was applied to analyze enumeration data between groups.And the Binary Logistic Regression was adopted to explore the relationship between the occurrences of Anemia in Lung Cancer and its related factors.Results:The registration form for all the patients with lung cancer in our Oncology Department from January1st,2010, to December31st,2011, was built;255cases were entered and240cases were qualified ones. The results were as follows. As far as general data was concerned,140cases were male, accounted for58.3%and100cases were female, accounted for41.7%. The average age was (65.04+10.24) years old for male and (64.64±12.11) years old for female. As far as information of the disease were concerned, the pathological types included41cases of squamous Cel1Carcinoma (17.1%),121cases of adenocarcinoma (50.4%),25cases of small cell carcinoma (10.4%), one case of adenosquamous carcinoma (0.4%), two cases of sarcomatoid carcinoma (0.8%), one cases of mucocellular carcinoma (0.4%), and49cases of unknown one (20.4%). For the stage of lung cancer, there were ten cases in stage I (4.2%), five cases in stage II (2.1%),44cases of stage III (18.3%) and181cases in stage IV (75.4%). For the metastasis,147cases were counted for mediastinal lymph node metastasis,11 cases for renal or adrenal metastasis,35cases for brain metastasis,81cases for Intrapulmonary metastasis,29cases for pleural metastasis,21cases for liver metastasis,67cases for bone metastasis,48cases for pleural effusion and10cases for pericardial effusion. Among the75cases of Anemia, accounted for31.3%, there were47cases of male and28cases of female, accounted for62.7%and37.3%respectively. As far as information of recent treatment were concerned, there were seven cases receiving operation,30cases receiving chemotherapy, sever cases receiving radiotherapy,33cases receiving target drugs,12cases receiving chest drainage and intracavity therapy and six cases receiving other therapies, including treatment for infection or bone metastases. As far as the occurrence of Anemia was concerned, the incidence of Anemia was33.6%for male and28%for female. There were122cases whose level of serum albumin was less than35g/L, accounted for50.8%, while61cases of them were diagnosed as Anemia, accounted for82.4%among all the cases of Anemia. As far as syndromes of TCM were concerned,52cases were diagnosed as the syndrome of blood stasis,184cases as the syndrome of phlegm dampness, six cases as the syndrome of water fluid,16cases as the syndrome of Qi stagnation,16cases as the syndrome of heat toxin,170cases as the syndrome of tumor-derived toxin,185cases as the deficiency syndrome of Lung Qi,150cases as the deficiency syndrome of Spleen Qi,12cases as the deficiency syndrome of Kidney Qi, six cases as the deficiency syndrome of Heart Qi, two cases as the deficiency syndrome of Kidney Yang, one case as the deficiency syndrome of Spleen Yang,65cases as the deficiency syndrome of Yin and33cases as the deficiency syndrome of blood. As far as the related factors for Anemia were concerned, the occurrence of Anemia in lung cancer was influenced by age (x2=7.394, P=0.007), the level of serum albumin (x2=40.787, P=0.000), recent chemotherapy (x2=4.657, P=0.031), the syndrome of tumor-derived toxin (x2=4.437, P=0.035), the deficiency syndrome of Lung Qi (x2=5.652, P=0.017) and the deficiency syndrome of Spleen Qi (x2=12.165, P=0.000), according to the results of the Chi-square test. Then the Binary Logistic Regression was adopted to further analyze the relevance between the occurrence of Anemia and recent chemotherapy (B=l.436, P=0.005), the level of serum albumin (B=1.944, P=0.000), the deficiency syndrome of Lung Qi (B=1.138, P=0.012), the deficiency syndrome of Spleen Qi (B=l.222, P=0.001) and the syndrome of tumor-derived toxin.(B=0.855, P=0.032).Conclusions:After the analyses for the240cases of primary lung cancer, the conclusions are drawn below. First, the factor of age influences the occurrence of Anemia, while the incidence of Anemia for patients older than60years old is higher than those less than60years old, but it is not a chief factor. Second, the level of serum albumin will influence the incidence of Anemia, while the incidence is higher for the level of serum albumin less than35g/L. Third, the relevance between Anemia and recent chemotherapy is significant, showing that the patients who received chemotherapy will suffer from higher incidence of Anemia compared wi th those who did not receive chemotherapy. Fourth, there were statistical difference between Anemia and the syndrome of tumor-derived toxin, the deficiency syndrome of Lung Qi and deficiency syndrome of Spleen Qi, while the existence of deficiency syndrome of Spleen Qi of Lung Qi will have more obvious effect on the occurrence of Anemia in lung cancer.
