On January 21st, the State Council held an executive meeting, examined and approved in principal“views on deepening the reform of the medical and health system”and“the implementation plan of deepening the reform of medical and health system from 2009 to 2011”, in April the same year,“the State Council’s views on the deepening reform of the medical and health system”was promulgated. This heralded a new round of the medical and health system reform. The general objectives of this round of deepening reform are :to establish a basic medical and health system that covers both rural and urban residents and provide the masses a safe , effective, convenient and inexpensive medical and health service. In order to realize these objectives, to improve the quality and potency of specialist physicians is an indispensable measure. Physicians are divided into two types: general practitioners and specialists. Today with the rapid development of medical standards, medicine now is divided into many specialists and sub specialists from non classification originally. In accordance with the classification code (classification and code of disciplines) People’s Republic of China National Standard (GB / T 13745292) and the "List of subjects in medical clinics" of Ministry of Health in 2003, there are 33 specialists and 109 sub specialists in China’s current clinical departments. The system of specialists is brought by the development of medicine towards specialization and is implemented comprehensively by many countries and districts; it has already been formalized and integrated into law. But in China it was only until 2006 that to carry out specialist training was clearly put forward. The Ministry of Health Office of Science and Education Guardian〔2006〕issued a document on the 27th that marks the release of the system construction of China’s specialist on the agenda. As an international city Shanghai is striving to become a“four centers”Shanghai will also host the 2010 world expo, all these pose higher requirements to medical and health development. As a very important link in medical development, special physicians’development is the problem shanghai’s medical cause has to face and solve. Currently the system of Shanghai’s specialist physicians is under construction and improvement. When this paper was close to ending in March 2009, Shanghai Health Bureau issued a "certification approach of specialist training base in Shanghai”which promoted the specialist system in Shanghai to move a step forward. But there are still a lot of problems in training system, transferring system and management system included in the specialist physicians system such as: the major problems existent in the medical education, the constraint of doctors title system, lack of entrance system for specialist physicians. On reviewing past relative literature and with the example of the advanced practical experiences in other countries and districts, this paper dose research on how Shanghai can improve its specialist physician system in line both with its reality and international standards, then puts forward some advice on how to reform and improve Shanghai medical education system, improve traditional physician title system and regulate the entrance system of specialist physicians based on there.
