| Objective: As the leptin has close relationship with lipid metabolismand bone metabolism, so it may be relates to the incidence of femoral headnecrosis. To study the correlation between serum leptin, serum lipid andosteonecrosis of the femoral head.Methods: From June2011to March2012,65patients withnontraumatic osteonecrosis of the femoral head were selected fromDepartment of Orthopedics, the Third Hospital of Hebei Medical University(Diagnosis standard reference ARCO stages), There were55male cases,10female cases. According to the reason of disease,65patients were dividedinto hormone group, alcoholic group, and diopathic group Inclusion criteria:①C onform to the diagnosis standard.②In the age of twenty to sixty yearsold.③T he Han nationality.④Native place in shijiazhuang City hebeiprovince and surrounding areas.⑤Engaged in light, medium strengthmanual workers. Exclusion criteria: Suffer from medical history such as highblood pressure, heart disease, diabetes and so on. Select sixty cases of healthpeople at outpatient service physical examination as control group(removethe patients who were taken adrenal cortical hormone andcholesterol-lowering drugs recently). The male24cases, female36cases.These2groups were extracted emptiness peripheral venous blood4ml, afterseparat serum, placed in the refrigerator with-80℃to test.The serum leptinlevel of two goups were detectived with radioimmunoassay. With fullyautomatic biochemical analyzer serum cholesterol, triacylglycerol, highdensity lipoprotein, low density lipoprotein, apoA1and apoB levels weretested. the results were analysised with statistical comparison.Results:①In osteonecrosis of the femoral head group, the levels of serum leptin(21.60±12.57μg/L), was significantly higher than that in thecontrol group(10.77±6.19μgap/L), the difference were significant(P<0.05).②I n osteonecrosis of the femoral head group, the level of Cholesterol,triacylglycerol, low density lipoprotein cholesterol(ldl-c) and apolipoproteinB(4.65±0.94mmol/L,1.61±0.83mmol/L,2.81±0.79mmol/L,0.94±0.23g/L),were significantly higher than those in the control group(3.92±0.71mmol/L,1.08±0.42mmol/L,2.21±0.62mmol/L,0.79±0.21g/L), the difference weresignificant(P<0.05). while the levels of high-density lipoprotein andapolipoprotein A1(1.11±0.33mmol/L,1.08±0.22g/L)were significantly lowerthan those in the control group(1.45±0.41mmol/L,1.43±0.30g/L), thedifference were significant(P<0.05).③According to the cause, Femoralhead necrosis group divided into there groups-hormone, alcohol andidiopathic. the levels of leptin, Cholesterol, three acyl glycerin, low densitylipoprotein, apolipoprotein B of the three groups were significantly higherthan those in the control group(P<0.05). the levels of leptin, Cholesterol,three acyl glycerin, low density lipoprotein, apolipoprotein B between thethere groups were no significant difference(P>0.05); While the levels ofhigh-density lipoprotein and apolipoproteinA1in the three groups weresignificantly lower than those of the control group(P<0.05). the levels ofhigh-density lipoprotein and apolipoprotein A1between the three groupswere no significant difference(P>0.05).④In osteonecrosis of the femoralhead group, the levels of leptin in female patients(28.76±16.22mmol/L)weresignificantly higher than that in male patients(19.40±10.36mmol/L). theyhave significant difference (P<0.05). In the control group the levels ofleptin in female patients(13.98±6.12mmol/L) were significantly higher thanthat in male patients(5.08±2.87mmol/L), they have significantlydifference(P<0.05).⑤I n osteonecrosis of the femoral head group,Thelevel of serum leptin was positive correlated with total cholesterol,r=0.61(P<0.05). The level of serum leptin was positive correlated with threeacyl glycerin, r=0.55(P<0.05). In the control group, The level of serumleptin was uncorrelated with total cholesterol, r=0.25(P>0.05), The level of serum leptin was uncorrelated with three acyl glycerin, r=0.30(P>0.05).Conclusion:①The increasing of the serum leptin level in patients withnontraumatic osteonecrosis of the femoral head plays a role in the process ofthe disease.②The serum leptin concentrations are positively correlated withthe development of nontraumatic osteonecrosis of the femoral head. |
