| BACKGROUND:The relationship between the occurrence time of bone marrow edema(BME) in nontraumatic osteonecrosis of femoral head,NONFH and clinical symptoms are still controversial.OBJECTIVE:To evaluate the relationship between ARCO international stage, pain grading and bone marrow edema in patients with nontraumatic osteonecrosis of femoral head(NONFH),which can improve the Clinical cognition of BME in NONFH.METHODS:NONFH in 43 case of 58 hips. According to T1 MRI weighted images and T2 fat suppression images, BME was confirmed. Bone marrow edema grading according to Zhao Pei-rong MRI Classification Method(grade I diffuse to the femoral head; grade II diffuse to the femoral head, femoral neck; grade III diffuse to the femoral head,neck,proximal femur). The disease staging according to the system of the Association Research Circulation Osseous(ARCO).Pain grading according to the pain project of Harris hip score. We analyze the correlation between BME and ARCO staging,pain grading.RESULTS AND C0NCLUSI0N:The incidence rate of BME in I-III ARCO stage was increased, and the difference was statistically significant(X2=27.69,P=0.001). The average rank of BME in I-III ARCO stage was increased, and the difference was statistically significant(X2=19.947,P=0.000). The incidence rate of BME increased among different pain grading, and the difference was statistically significant(X2=57.0,P=0.000). The average rank of BME grading among the pain grading was increased, and the difference was statistically significant(X2=174.0,P=0.000).CONCLUSION: The presence and aggravation of BME indicated the increase of NONFH necrosis and the aggravation of symptoms. Can be used as an index for the appraisal of advancement of disease. |
