Objective1. To analyze higher-order aberration which influenced the height of refraction map (HRM) and investigate the relationship between the HRM and modulation transfer function (MTF) in eyes implanted intraocular lens (IOL) at3mm,4mm,5mm pupil diameter.2. To Explore the effect of the height of refraction map on the depth of focus and stereoscopic vision at intraocular lens eyes after phacoemulsification sugery.Methods1. Clinical case self-control study. One hundred and ten age-related cataract patients(120eyes) who were operated with phacoemulsification and IOL implantion were enrolled. The i-Trace was used to measure the higher-order aberration,MTF, refraction map. The image-pro-plus6.0software was adopted to analyze the height of refraction map (HRM), P<0.05for the difference was statistically significant.2. Clinical case self-control study. Ninty age-related cataract patients (150eyes) who were operated with phacoemulsification and IOL implantion were enrolled. The i-Trace was used to measure the higher-order aberration, MTF, refraction map. The image-pro-plus6.0software was adopted to analyze the height of refraction map (HRM). After fully paralyzing ciliary muscle, use a4mm Artificial pupil to confine the diameter of patient’s pupil size. Check postoperative patients with phoropter with logMAR visual chart and draw the defocal curve from the results. Divided the patients into three groups according to the height of refraction map. Adopt the single factor analysis of variance to analyze the data. P<0.05for the difference was statistically significant. Pick60cases (120eyes) which postoperative patients’stereoscopic vision can be detected, and divide them into two groups according to the height of refraction figures. According to the international standard, divide them into normal and abnormal groups, and then gain each group’s figures. Adopt Chi square test to analyze the data. P<0.05for the difference was statistically significant.Results1. The HRM increased with pupil diameter, and MTF decreased. The HRM was positive correlation with coma (r=0.784,P<0.05),The HRM was not correlation with spherical and trefoil aberration at3mm pupil diameter. The HRM was positive correlation with coma (r=0.627,P<0.05), The HRM was not correlation with spherical and trefoil aberration at4mm pupil diameter.The HRM was positive correlation with spherical aberration (r=0.358,P<0.05), The HRM was not correlation with coma and trefoil aberration at5mm pupil diameter. The MTF(tHOA-15c/d) was negative correlation with HRM (r=-0.606,P<0.05) at3mm pupil diameter and4mm pupil diameter(r-0.335,P<0.05),while there was no correlation at5mm pupil diameter.2. When the pupil diameter is4mm, the mean depth of focus which corresponds to logMAR0.1is (1.70±0.30) D; the mean depth of focus which corresponds to logMAR0.2is (2.21±0.46) D; the mean depth of focus which corresponds to logMAR0.3is (2.77±0.67) D. The differences of three groups’DOF which correspond to logMAR0.1were statistically significant (F=10.188,P=0.001); The differences of there groups’DOF which correspond to logMAR0.2were statistically significant (F=19.052,P=0.000);The differences of there groups’DOF which correspond to logMAR0.3were statistically significant (F=29.509,P=0.000); The differences of two groups’Stereoscopic vision、 Crossed and uncrossed disparity were not statistically significant.Conelusion1. When pupil diameter were3mm and4mm, the main aberration influence was coma. When pupil diameter was5mm, the main aberration influence was spherical aberration, When pupil diameter were3mm and4mm, HRM can decline MTF (t HOA-15c/d).2. When the pupil diameter is4mm, the height of refraction map (HRM) can influence the subjective DOF. With the increase of the HRM, the DOF also increased. While the HRM cannot effect the stereoscopic vision、Crossed and uncrossed disparity.
