Objective: artificial femoral head replacement with dynamic hip screw((dynamic hip screw DHS) is fixed in the treatment of the oldest old intertrochantericfracture efficacy, and explore two methods to adapt to levy and contraindications.Methods: A retrospective analysis of our hospital from February2008toNovember2012.57patients hemiarthroplasty and dynamic hip screw fixation of twosurgical treatment elderly intertrochanteric fracture,There were25males and32females, aged80-94years old, the average age of85.3years, the hemiarthroplastygroup of30patients, DHSgroup of27patients. Followed up for6months to2years,an average of15months, the gender of the two groups of patients were recorded, age,type of fracture, surgery time and amount of bleeding; the Shimoji time of thefollow-up of patients, the incidence of postoperative complications or mortality andintraoperative and poor pre-and postoperative Harris hip score, and the two methodsused for statistical analysis and comparison.Results: hemiarthroplasty and dynamic hip screw fixation group were followedup for6months to2years, with an average of15months. Internal fixation group inthe the hemiarthroplasty group with DHS in operative time, bleeding was nosignificant difference (P>005); while the former earlier postoperative ambulationtime, lower the incidence of surgical complications, hip recovery of joint functionbetter, both the difference was statistically significant (P <0.05).Conclusion: Dynamic hip screw fixation and hemiarthroplasty are effective intreating intertrochanteric fractures, both in terms of operative time and bleeding thedifference was not statistically significant, next to the time and postoperativecomplications and hip recoverysuch as the difference was statistically significant comparative advantages hemiarthroplasty, hemiarthroplasty was significantly shortertime in bed for the elderly, to reduce the incidence of postoperative complications inpatients with early ambulation the patient’s hip functionhave also been improved, webelieve that the elderly intertrochanteric fracture, hemiarthroplasty in the earlyambulation to prevent related complications.
