| Objective:To compare the curative effects of bipolar hemiarthroplasty(BHR) and dynamic hip screw(DHS) for unstable intertrochanteric fractures in elderly patients.Methods:The clinical and radiological records of 60 elderly patients with femoral unintertrochanteric fracture treated by BHR(18 cases)and DHS(42 cases) from January 2003 to January 2009 were retrospetively reviewed. All these patients were followed-up for 12 months. For time of operation, blood loss in operation, incidence of complications, time of postoperative of weight-bearing and the Harris's hip functional standard scoore were compared and analyzed.Results:There were significant difference between the time of operation [BHR group mean operation time was 66±18min, (range 45-90 min), DHS group 96±32min, (range 45-190 min)] and time of postoperative of weight-bearing [BHR group was 13±4d, (range 8-21d),DHS group 49±12d, (range 23-75d)](all P<0.01),According to the Harris's hip scoore. There were significant difference between two groups in the Harris's hip scoore [BHR group 92±5.3 points,(range 78-98 points)) DHS group 83.5±11.1 points, (range 46-97 points)](P<0.01), the rate of excellent and good in BHR group was 94.4%, Which was higher than of DHS group 69%,(P<0.05).There were significant difference between two groups in the incidence of complications (BHR group 16.7%,DHS group 47.6%)(P<0.05), The mean blood loss in operation[BHR group 163±149ml, (rang 50-700ml) DHS group 211±107ml, (rang 50-500ml)] there were no significant difference(P>0.05).Conclusion:Through the retrospective study of elderly patients with femoral unintertrochanteric fracture treated by BHR and DHS. BHR compared with DHS inernal fixation,The BHR advantages of shorter operation time, allow early weight-bearing, little complication and higher of the Harris's hip scoore, is one of a better alternative treatments to the aged patients with unstable intertrachanteric fractures, Long-term effect needs further observation.
