Object:Disccuss therapeutic Mechanism of "Tong mai Granule" to qi deficiency and blood stasis syndrome in carotid artery atherosclerosis patients through clinical observation of the Chinese herb Granule’s influence on blood fibrinogen.Methods:77carotid artery atherosclerosis patients (matched with western medicine and TCM syndrome differentiation diagnostic criteria) in a randomized parallel controlled study in Xiamen traditional Chinese medicine hospital have been enrolled in our research After getting1-77randomized drug flow Numbers by SAS statistical analysis system, eligible patients were divided into two groups according to their doctor order, half of them (39,group1) received2bags of tong mai Granule twice a day with blunt water for8weeks, whereas38of them (group2) randomly assigned to receive100of Aspirin Enteric-coated Sustained Release Tablets and20mg of atorvastatin per night using consecutively8week. All patients received Carotid ultrasound、coagulation function (fibrin original value, part of the blood coagulation time live enzyme, prothrombin time), liver and kidney function, serum lipid, carotid artery ultrasound, blood routine, defecate normal, defecate occult blood, urine and TCM symptom scores detection. Results:There is no significant difference (P>0.05) in blood fibrinogen, carotid artery atherosclerosis degree, serum lipid and TCM symptoms score between two groups before treatment. Blood fibrinogen decreased significantly in both groups from4weeks after treatment(P<0.01), and from8th week, there is statistic difference between tong mai Granule group and western medicine group(P<0.01). Degree of carotid artery atherosclerosis, part of the blood coagulation time live enzyme (ATTT) and prothrombin time (PT) had no obvious change (P>0.05) either4weeks or8weeks after treatment in both groups, and had the same result when compared between groups. Serum lipid significantly decreased in western medicine group(P<0.01)and had statistic difference when compared to tong mai Granule group (no statistic change in this group,P>0.05) from4weeks to8weeks after treatment. Treatment effects of the two groups are as follows:Both tong mai Granule group and western medicine group reduce patients fibrinogen, obviously. Both groups can improve TCM symptoms, the total effective rate of tong mai Granule group is79.48%, statistically higher (P<0.01) than western medicine group’s39.47%.Conclusions:1. Tong mai Granule can reduce blood fibrinogen in carotid atherosclerosis patients with qi deficiency and blood stasis.2. Tong mai Granule can improve clinical symptoms in carotid atherosclerosis patients with qi deficiency and blood stasis.3. Degree classification of carotid artery atherosclerosis is not obviously improved after8weeks treatment of Tong mai Granule.4. Both two groups doses not appear adverse reaction and side effects In the treatment of doses, liver function and creatine kinase has no obvious rise before and after treatment.
