Objectives:We investtigated the prevalence of H.pylori in patients with liver diseases, compared the prevalence of hepatitis B, hepatic carcinoma, liver cirrhosis and other diseases. We discussed the relationship between H.pylori and chronic liver disesses.Methods:556patients with liver diseases were collected and detected anti-H.pylori IgG in their serum. Positive results mean the patients were infected by Hp. So we can realize the infection of Hp in those patients.20patients with primary hepatic carcinoma,7patients with gastric carcinoma,4patients with lung cancer,4patients with cholangiocarcinoma,5patients with rectal carcinoma were collected, who were all operation samples and diagnosed by histopathology. We extracted DNA from those samples and examined Helicobacter pylori by polymerase chain reaction(PCR) using Helicobacter-specific16S rRNA gene and the special26-kDa gene of Helicobacter pylori as primers. Those specimens were all performed immunohistochemical staining for H.pylori.Results:The prevalence of H.pylori infection in patients with hepatitis B、HBV-related hepatic carcinoma and HBV-related liver cirrhosis was62.7%,77.0%,79.7%respectively; The prevalence of Hp infection in patients with non-hepatitis B liver diseases is44.5%; Compared to the patients with non-hepatitis B liver diseases, significant difference was found between two groups(P<0.001). The prevalence of H.pylori infection in patients with HBV-related hepatic carcinoma and HBV-related liver cirrhosis was higher than the prevalence of H.pylori infection in patients with Hepatitis B(P<0.05). The prevalence of H.pylori infection in patients with HBV rising was high, but the prevalence was similar in patients with different levels of HBV virus. The prevalence of H.pylori infection in patients with Child-Pugh classes A, B, and C liver cirrhosis was51.9%,63.4%,65.6%. There was no significant difference in patients with different grades of liver cirrhosis.45.0%samples of hepatic carcinoma were positive for helicobacter-specific16S rRNA gene, but none was positive in other cancers except gastric carcinoma(P<0.05). Of these positive samples,66.7%samples were positive with the26-kDa primers of H.pylori. Curved or bacillus bacterias were detected in liver tissue of patients with primary hepatic carcinoma(30.0%), but no bacteria was found in other cancers excpet gastric carcinoma(P<0.05).Conclusions:The prevalence of H.pylori infection in patients with chronic liver disease leaded by HBV was higher than that in other liver diseases. The patients with hapetitis B who also had got hepatic carcinoma or liver cirrhosis were more easily infected by Hp. Maybe the infection of Hp has relationship with the complication of HBV-related liver diseases; and Hp may have toxicity for hepatocyte. The Helicobacter species can be found in the liver tissue of patients with primary hapetic carcinoma, and it is possibly linked to carcinogenesis of hepatic carcinoma.
