Research On The Stratified Model Of Disease-oriented Outpatient In Comprehensive Hospitals

Accompany with the society develop rapidly, people’s requirement for medical services is also getting higher and higher, traditional out-patient model has been unable to fully meet the medical needs of patients with personality. Secondly, medical technology advances by discipline breakdown is its notable feature. In this case, general hospitals adapt the competition in team medical to meet the growing patients’ overall health care needs, but also discipline constant must be continuous breakdown in order to promote the medical technology develop. So as to provide humanity and careful service to the patient, and the disease-oriented outpatient comes out by this background.Disease-oriented outpatient has become a hot spot in recent years for comprehensive hospital innovating their service model,, a variety of disease-oriented outpatient springing up in the majority of general hospitals, and in the ascendant. However, this model are still new, and lack of theoretical support, neither unified management system recognized and of standards, etc., As the best run state of this model, different hospitals is also a heartfelt issue. Lead to various hospitals to open disease-oriented outpatient quality varies greatly, and patients with mixed reviews. In addition, the literature search, not yet see any system research of this model, I saw some literature introducing their experience and practices, theoretical research is clearly lagging behind the practice to explore. To investigate to solve the above problems, we address this research, in order to provide meaningful reference and theoretical guidance for the comprehensive hospitals of practicing.This thesis can be divided into three parts in seven chapters, the first part include introduction and first chapter, which discusses the background of the disease-oriented outpatient model appears, summarized and presented the concept of disease-oriented outpatient, and from the development of disease diagnosis and treatment patterns all over the world, explains why the general hospital in China set up disease-oriented outpatient. The second part is the core part of this paper, including the second and third chapters, we system studied the theoretical framework of the special disease outpatient model and its auxiliary conditions for building. The third part is to validate its comprehensive benefits, including chapters4and5, in addition to the benefit analysis theory, this study analyzed and research the two kinds of disease-oriented outpatient each by a typical case, and present a survey to probe patients’recognition of the model, demonstrated the comprehensive benefits of the model. The last Chapter6is the conclusion, which draws the final conclusion of this study, and discusses the future trend of disease-oriented outpatient mode.First is the introduction, this paper begin with advances in medical science and a few obvious trends, combined with the mainly confusion when people seek medical treatment, and summarized its concept.Chapter Ⅰ the current situation of the disease-oriented outpatient model. In an overview of the disease treatment model of foreign hospitals, summary of three models of common out-patient clinics, specialist clinics and disease-oriented outpatient. And through the Internet homepage of80large hospitals, we collected the creation of the disease-oriented outpatient condition of each hospital, summed up the situation we facing domestic, analyze the problems which exist on the need for general hospitals to open disease-oriented outpatient.Chapter Ⅱ is the construction of stratified model of disease-oriented outpatient. Firstly, classify this model into single-disciplinary and multi-disciplinary disease-oriented outpatient. Subsequently, from the management system and principle to construction, we system studied the theoretical framework of the model, that the disease-oriented outpatient must have a sound management system, such as access to physicians, quality control, distribution of benefits, etc. And it shall comply with principles, such as the principle of special, overall planning principles, characteristics principles, and pay attention to the problems in actual operation.The Ⅲ chapter is the auxiliary conditions for the construction of the disease-oriented outpatient. Information technology and digitization of the basic features of modern hospitals, disease-oriented outpatient needs the support of a number of conditions, this chapter discusses it from the facts of doctors’workstations, database construction, advertisement support. The second section examines the medical data application in medical research and teaching.Chapter Ⅳ is typical cases of disease-oriented outpatient. In this chapter, through the two different typical and successful cases, we further explore the theoretical status of the model. Selected Nephrology Center of Nanjing General Hospital, as the success case of the practice the single-disciplinary model, and Beijing Union Medical College Hospital as a practice of multi-disciplinary model. The two cases with strong representation can be used as a typical sample to explore the ideal state of the disease-oriented outpatient model.Chapter Ⅴ is the benefit analysis. First from six aspects of convenience for patients, to promote rational health care, to create the featured brands etc, analyzed its importance to hospital strategic development. Subsequently, to test the overall efficiency of the model, we choose Nephrology Center of Jingling hospital as a typical object of study, presented a disease-oriented disease outpatient cognitive questionnaire. This questionnaire was made after extensive consultation with relevant clinical and hospital management experts, two sets of questionnaires were developed for newly diagnosed patients and patients of disease-oriented outpatient questionnaire. Totally of1000questionnaire papers were distributed in both area, the conclusion found a more powerful proof of the final conclusions that this model facilitate patient treatment, deliver specialized medication, reduce the cost of this study, and so on.Chapter Ⅵ is the conclusion, the conclusion appears that, the disease-oriented outpatient model made people get treatment more convenient and specialized. To some extent, it can solve the difficult and expensive problems for patient to obtain a doctor, and it also conducive to the hospital discipline construction and personnel training. Finally, discussed the new model’s future development trends, confirm that, the model will become the hot spot for hospitals to innovation its’service.In this study, methods of literature search, case study method, questionnaire survey and interview a variety were integrated used. Specific technical routes as follows:First, from literature search, we systematic studied hospitals’outpatient service models, doctors training models, disease-oriented treatment model, disease-oriented outpatient model and research-related content, inspired by the foreign hospital, and on the basis of experts interview, we explore and build the model structure of the mode of disease-oriented outpatient, and enrich it with theory and practice contents. Secondly, through case study, to infer its ideal state, and key issues should be addressed. Finally, further use a series questionnaire survey to probe the patient’s overall point of view. In order to demonstrate it brings convenience and benefits to patients, and draw the final conclusion of this study.This research draws that disease-oriented outpatient will be the hot spot growing spot for hospitals to innovate its service model. Secondly, this model can guarantee patients’ safe, high-rate satisfaction; can promote the level of the medical service. Additionally, the model also can improve medical quality, decrease medical cost.The specialty of this study is that, it firstly addresses the concept of disease-oriented outpatient, and define its contents. Secondly, theory probe the ideal state of the disease-oriented model from four aspects of creation discipline, management mechanism, etc, to provide meaningful guidance to hospitals’practice. And provide a new way of thought for hospital to strengthen talent training, so as to build brand. Lastly, confirmation the effective of the stratified model of disease-oriented outpatient.This study has some certain reference value for hospitals’ practice of the new outpatient service mode, but it also got its own basic law, hospital should adjust it according to their own circumstances, contingency to determine, because not all the comprehensive hospitals, or all disciplines are appropriate to open disease-oriented outpatient. Secondly, in the analysis of its comprehensive benefits, this study lack of demonstration of the effect from the aspect of hospital, neither the point of view of the medical staff, but does not affect the final conclusions of this study.
