Objective:The Fu Zheng Xie Zhuo Bao Shen Decoction, Professor Ma Juli’s experienced decoction, is used to treat the chronic renal failure (CRF) in the early and mid-term stages in clinic, which has proved the therapeutic effects is significant after over forty years’clinical practice. This subject aims to discuss its effect on Smad4and Smad7signal pathway by observing the therapeutic effects of the prescription to the adenine inducing CRF rat model and provide evidences for better clinical application.Methods:Selecting10from50SD healthy male rats randomly as the blank group. The rest40were intragastric administration, which were used to build CRF rat model. After that, the40rats were randomly divided into four groups: the model group, the Losartan group, the low-dose group (18.8g/kg), high dose group (37.6g/kg), each group with10rats. The rats in the low-dose group and high dose group were intragastric administration by Fu Zheng Xie Zhuo Bao Shen Decoction for30days and compared with the rats in the blank group, the model group, the Losartan group. Determine Scr and BUN of the serum, as well as red blood cell (RBC), hemoglobin (Hb). Determine the level of Smad4and Smad7in the kidney tissue of the rats by the method called immunohistochemical half quantitative he pathological change of the kidney. Results:The level of Smad4, Scr, BUN and24h urinary protein is significantly higher than that in the blank group (p<0.01) and lower than that in the high and low dose group, the Losartan group (p<0.05). Compared with the low dose group and the Losartan group, the level of them decreased significantly in the traditional Chinese medicine group (p<0.05). There is no significant difference between the low dose group and the Losartan group (p>0.05). The level of Smad7, RBC, Hb in the model group was significantly lower than that in the blank group (p<0.01). Compared with the model group, the level of them increases in the high and low dose group, the Losartan group (p<0.05). And the increases is apparent in the Chinese Medicine high dose group (p<0.05). There is no significant difference between the low dose group and the Losartan group (p>0.05).Morphologic observation of kidney tissue:the renal corpuscle, kidney tubules and renal interstitium of the rats are normal in the blank group. The decrease in the number of renal corpuscle, partial atrophied renal corpuscle, focal and flake necrosis in the proximal tubular epithelial cell with lymphocyte infiltration, kidney tubules with particle tube, partial cystic dilation of the kidney tubules and apparent hyperplasia of interstitial connective tissue are see in the model group, as well as normal blood vessels. A slight decrease in the number of renal corpuscle, few partial atrophied renal corpuscle, focal and flake necrosis in the proximal tubular epithelial cell with lymphocyte infiltration, few fallen off epithelial cell and cast in the kidney tubules, occasional partial cystic dilation of the kidney tubules and slight hyperplasia of interstitial connective tissue are see in the Losartan group, as well as normal blood vessels. There exists little difference between the low dose group and the Losartan group. There is no significant decrease in the number of renal corpuscle, and no significant pathological change in the kidney tubules, but occasional atrophied renal corpuscle and cast in the kidney tubules, unconspicuous hyperplasia of interstitial connective tissue, normal blood vessels can be seen in the high dose group. Conclusion:This project work demonstrates that the Fu Zheng Xie Zhuo Bao Shen Decoction can improve the mental state, food intake, water drinking, weight gaining and living condition of the rats effectively. What’s more, it can decrease the level of Scr, BUN and24h urinary protein, increase the level of RBC、Hb and improve the pathological impaired kidney tissue, whose mechanism is achieved by inhibit the frequent expression of the transitional growth factor β (TGF-β) downstream factor Smad4and improve the negative feedback expression of the Smad7to prolong the kidney impairment.
