Abstract/Summary:
Background and purpose:many studies have proved that,there exists interaction in pathology and physiology of the kidney,modern medicine in pathology of heart and kidney syndrome is defined,and made a further exploration of the pathogenesis,diagnosis and treatment,and has put forward the theory of ancient medicine China heart and kidney related to discuss.In recent years,due to the interaction of heart and kidney increased the development of autoimmune disease,which suffers from kidney disease or heart disease mortality is increasing,heart and kidney related research more and more attention,and has important clinical significance.Based on the study of the incidence,risk factors,and the rate of decline in GFR relationship between theoretical research of traditional Chinese medicine in patients with chronic renal insufficiency secondary to heart failure,further use of scientific data confirm the scientific nature of Chinese medicine "heart and kidney related theory,and in the evaluation of heart failure,the clinical commonly used screening index:BNP specifically,on patients with chronic renal insufficiency secondary to BNP>400ng/L when the incidence,risk factors,and the relationship between the degree of reduced glomerular filtration rate.Methods:the data of the study were selected from November 2015 to-2016 in the Second Affiliated Hospital of Dalian Medical University in the Department of nephrology in,and diagnosed as chronic renal insufficiency in the CKD4-5 phase of the patients in the 672.According to the results of BNP,400ng/L as the threshold evaluation of heart failure,BNP>400ng/L elevated group,on demographic characteristics,medical records of all patients and laboratory test results were collected,in the relevant data analysis,data classification with frequency(100%),chi square test with continuous data processing;with the mean + standard deviation,using independent samples t test,the data processing using statistical software SPSS22.0.Result:1.The incidence of BNP>400ng/L in patients with chronic renal insufficiency(294/672)was 43.75%.2.1n all patients,diabetic nephropathy accounted for the first cause of chronic renal failure,and the incidence of BNP>400ng/L was the highest,which was 59.16%.3.There was no significant difference between the two groups in terms of gender,dialysis treatment,and in-hospital mortality among all the groups with or without elevated BNP(P>0.05).4.in all groups analysis,there is no comparison between the two groups of BNP increased,there are significant differences between the chronic renal insufficiency in CKD4 and CKD5 phase(P<0.05),CKD5 patients with increased BNP incidence was significantly higher than that of CKD4 patients.5.in all patients,the risk factors of BNP include increased age,serum total protein(TP),calcium(Ca),triglyceride(TG),sodium(NA),serum albumin(ALB),serum iron(SI),iron saturation(TS),fasting blood glucose(FBG).6.in the subgroup of patients with diabetic nephropathy and chronic glomerulonephritis,BNP increased risk factors include age,hemoglobin(Hb),triglyceride(TG),fasting blood glucose(FBG),parathyroid hormone(PTH);for the subgroup analysis of anemia,elevated BNP risk factors include age,fasting blood glucose(FBG)sodium(NA),triglyceride(TG),serum albumin(ALB);for the subgroup analysis of hyperuricemia,the risk factors of BNP include increased age,serum total protein(TP),triglyceride(TG),serum albumin(ALB).7.the cause of death subgroup analysis,the proportion of sudden cardiac death,which is about 36.84%,followed by uremia encephalopathy and shock accounted for about 15.79%.Conclusion:1.The incidence of BNP>400ng/L in patients with chronic renal insufficiency in CKD4-5 stage was 43.75%.2.Independent risk factors of age,triglyceride BNP>400ng/L.3.BNP>400ng/L and glomerular filtration rate decreased,the incidence of patients with stage CKD5 was significantly higher than that of the CKD4 phase,and the more serious the kidney itself,the greater the impact on cardiac function.4.chronic renal failure patients with secondary heart failure is widespread,further confirmed the Chinese medicine "heart kidney related" theory of science.
