Background and purpose:Cardiovascular disease has become the most threatening health problem of human-beings in the world. The risk factors include aging, obesity, hypertension and diabetes, etc. The structure and function of cardiovascular system varies with aging, so it’s basically important to discriminate the pathophysiologic state from aging. Through almost half a century’s scientific researches and clinic applications, echocardiography has become a routine method to investigate the cardiac structure and function, as well as the hemodynamic status. It has been accepted by medical workers for its invasive and economic. However, the normal range of echocardiography was formed in the1980’s, and people’s nutritional condition and lifestyle has changed a lot, the old standard is no longer suitable. It is of great scientific urgency to establish the normal range of echocardiographic values of normal Hunan adolescences and adults and to determine the influence of aging and surface area on the values. The physiological changes of left ventricular systolic and diastolic functions were also discussed. Our research was managed to perform a preliminary study prior to a large-scale population research of echocardiographic measurements of normal population in Hunan province.Objects and methods:Five hundred and seventy-eight cases of normal han nationality adolescences and adults were examined from July2011to April2012in the2nd xiangya hospital of central south university hospital.312cases were male,266cases were female with their ages ranging from14to77year-old (65.5±3.2years). There are124cases in the age group of14-17.143cases in the age group of18-40.172cases in the age group of41-65. and139cases in the group of65and above. We used a Philips iE33color Doppler ultrasound device with a sector array1-5Mhz probe. The left edge of sternum, the apex, the concave, the sword on sternum or sides and the sternal notch and the sword or ribs were selected as testing area. Statistical analysis of age, sex and surface area with echocardiographic test result were carried out. Furthermore, inter-observer and inter-observer repeatability tests were also performed in the first50people.Results:AoD, LAD, LVEDD, LVESD, LVPWd,LVPWs and IVSd in male was higher than those in female. The difference is significant (P<0.05), within which AoD and LAD was the most dominant. There was no obvious relation between different genders in PAD, RVAWd, RVDD and IVSs (P>0.05). There was a positive relation in AoD, LAD, LVESD, LVPWd, LVPWs, and IVSs with the age (P<0.05),within which AoD and LAD was the most dominant. These was no significant relation in PAD, RVAWd, RVDD, LVEDD and IVSd with age (P>0.05). Among groups of different body surface area, there was statistical difference in AoD, LAD, LVEDD, LVESD, LVPWd, IVSd, IVSs and PAD(P<0.05) while no statistical difference in LVPWs, RVAWd and RVDD. The Left ventricular systolic function has no significant relationship with age,but its diastolic function was weakened with aging. The repeatability test showed a positive result in both intra-observer and inter-observer.Conclusion:1Our research preliminarily set up the normal range of echocardiographic measurements of normal Hunan adolescences and adults.2In left heart related indices, AoD, LAD, LVEDD, LVESD, LVPWd LVPWs and IVSd was related to gender, AoD,LAD, LVESD, LVPWd, LVPWs, and IVsd was related to age, so a age-specific and gender-specific normal range was needed for those indices. Whereas, there was significant relationship between right heart related indices and age and gender, so it was unnecessary to take those factors into concern for those indices.3Body surface area has influence in most measure data, so when we set up the echocardiography reference, body surface index should be take into consideration. 4The Left ventricular systolic function has no significant relationship with the age, but it’s diastolic function is weakened with aging.So we should carefully distinguished diastolic function’s weakening with aging and the diastolic heart failure.
