Detection Of Thyroid Function During Pregnancy And Normal Range In Wuxi Liangxi Area

?Objective?To establish the normal reference range of thyroid function in different stages of pregnancy in this region,and to provide the basis for the diagnosis of thyroid disease during pregnancy.?Methods? From June 2016 to December 2016,there were 540 cases of normal maternal serum samples from obstetrics and gynecology clinic in Wuxi Maternal and Child Health Hospital,excluding individuals with family history and family history of thyroid disease,excluding visible or accessible goiter,Exclude medications,excluding thyroid autoantibody positive.(Pregnancy 13 ~ 6 weeks ago),middle pregnancy T2 group(pregnancy 14 ~ 27 +6 weeks),late pregnancy T3 group(28 to 40 weeks of pregnancy)a total of 4 groups,The thyroid function(TSH,FT3,FT4)was detected by competitive immunoassay with direct chemiluminescence(LGA)using three diffrent kits(Abbott Architect I2000,Roche Cobas Elecsys 600 and ADVIA Centaur? CP kit,respectively,using reagent A,reagent B,reagent C,respectively.)According to the United States Institute of Clinical Biochemistry recommended thyroid function test indicators to determine the scope of the development method,the results of the results of the 2.5 to 97.5 percentile as the reference range of the indicators.?Results? 1.The reference range of TSH in Abbott's serum was 0.78 ~ 4.83 m IU / L,0.03 ~ 4.26 m IU / L,0.09 ~ 3.97 mIU / L,0.64 ~ 5.28 mIU in the early stage of pregnancy,the middle of pregnancy and the third trimester of pregnancy / L.The reference range of TSH serum TSH was 0.89 ~ 4.95 m IU / L,0.05 ~ 4.58 m IU / L,0.12 ~ 4.23 mIU / L,0.74 ~ 5.53 mIU / L in early pregnancy,middle pregnancy and late pregnancy.The reference range of TSH serum TSH was 0.71 ~ 4.78 mIU / L,0.03 ~ 4.19 mIU / L,0.07 ~ 3.86 mIU / L,0.62 ~ 5.12 mIU / L in early pregnancy,early pregnancy and late pregnancy.2.The reference range of Abbott serum FT3 was 3.68 ~ 5.86 pmol / L,3.51 ~ 5.62 pmol / L,3.07 ~ 5.39 pmol / L,2.63 ~ 4.76 pmol / L in early pregnancy,middle pregnancy and late pregnancy.The reference range of Roc's serum FT3 was 3.73 ~ 5.95 pmol / L,3.68 ~ 5.80 pmol / L,3.12 ~ 5.53 pmol / L,2.74 ~ 4.98 pmol / L in early pregnancy,early pregnancy and late pregnancy.The reference range of serum FT3 was 3.58 ~ 5.66 pmol / L,3.37 ~ 5.39 pmol / L,3.11 ~ 5.13 pmol / L,2.70 ~ 4.72 pmol / L in the early stage of pregnancy,the middle of pregnancy and the late pregnancy.3.The reference range of serum FT4 was 11.78 ~ 22.03 pmol / L,11.43 ~ 22.62 pmol / L,9.39 ~ 16.75 pmol / L,8.66 ~ 15.30 pmol / L in the early pregnancy,the middle of pregnancy and the late pregnancy.The reference range of Roc4 serum FT4 was 12.02 ~ 21.79 pmol / L,12.86 ~ 22.38 pmol / L,9.54 ~ 16.88 pmol / L,8.71 ~ 15.53 pmol / L in early pregnancy,early pregnancy and late pregnancy.The reference range of serum FT4 in pregnant women before pregnancy,early pregnancy,mid-gestation and late pregnancy was 11.61 ~ 21.47 pmol / L,11.53 ~ 22.79 pmol / L,9.07 ~ 16.75 pmol / L,8.50 ~ 15.12 pmol / L.4.The changes of TSH values of the three kinds of test reagents were the same at the time of 13 weeks and 16 weeks before pregnancy,which was significantly lower than that of other pregnancy points(P <0.01),and significantly higher than that of other pregnancy(P <0.01);TSH values between 14 ~ 27 + 6 weeks of pregnancy were between the two levels of pregnancy.There was no significant difference in P2.5th and P97.5th between the two groups(P> 0.05).The P53.5th values of the three syndromes were significantly higher than those of the American Society of Thyroid Society(ATA)and the reference range recommended by the Chinese Medical Association for Pregnancy and Postpartum Thyroid Diseases in China.The two were statistically different(P <0.01).5.The upper and lower limits of FT3 and FT4 decreased with the increase of gestational age.The FT4 and FT4 upper limit values of different reagents were different,the lower limit of FT4 and the upper and lower limits of FT3 were the highest,the upper limit of FT4 was detected by Abbott reagent,and the result of Siemens reagent was between the two.?Conclusion?1.The normal reference range of pregnancy-specific thyroid function in Abbott,Roche and Siemens kit was higher than that recommended by the American Society of Thyroid Society(ATA)and the recommended guidelines for diagnosis and treatment of pregnancy and postpartum thyroid disease in China.2.Consider the different living habits,dietary intake of different iodine,the experiment used in different reagents.Under recommended conditions,laboratorys in accordance with the United States Institute of Clinical Biochemistry(NACB)and the Chinese Medical Association of pregnancy and postpartum thyroid disease diagnosis and treatment recommended by the screening method to develop local normal pregnant women specific thyroid function reference range.
