Objective: Chronic congestive heart failure is a complex clinical syndrome, and thefinal stage of various heart diseases. Through animal experiment and clinical research, theresearch studies the Fuxin Tang anti heart failure mechanism from the neuroendocrine andmolecular biological level, providing a scientific basis for further clinical application.Methods: This study is divided into2parts:animal experiments and clinical research.1. Animal experiment: Use adriamycin cardiotoxicity to induce rat heart failure model.60male wistar rats are randomly divided into5groups: normal control group, model group,captopril group, Fuxin Tang low dose group and Fuxin Tang high dose group,12of eachgroup. The spirits, activities, eatings and color pattern are observed during the labprocedure. Six weeks later, the heart function of rats are determined, serum TNF-a levelsare detected by ELISA method, while pathological changes in cardiac muscle by HEdyeing and expressions of protein Caspase-8、Bcl-2in cardiac muscle of rats by immunitymethods are observed. All of the outcome set up a database to analyze in the way of singlefactor analysis of variance and statistical analysis with the SPSS16.0statistic software.2.Clinical research: According to the difference of age, sex and grade of heart function,30heart failure patients are randomly divided into the control group and treatment group,15cases in each group. The plasma BNP, life quality and drug safety are observed. All of theoutcome set up a database to analyze in the way of T test and X2test,statistical analysiswith the SPSS16.0statistic software.Results:1. Animal experiment result:①General situation of rats: Compared withnormal control group, model group rats gradually appear myocardial damage symptoms,manifeste as dispirited spirits, less activities, settled, standing reliance the cage; nasalbleeding, varying degrees of asthma sputum; diet and water reduced, diarrhea, weight loss, and ascites formed gradually; lose of fur,and different hair color, part of the rats appearpatchy hair removal, skin ulcers and necrosis. Compared with the model group, treatmentgroups have the similar symptoms later, and lighter.②Rats cardiac function: Comparedwith the normal control group, model group rats average LVSP and±dp/dtmaxdecreasesignificantly (P<0.01), while the average LVDP increased significantly(P<0.01). Comparedwith the model group, LVSP, LVDP,±dp/dtmaxof Fuxin Tang low dose group show nosignificant differences (P>0.05); while Fuxin Tang high dose group show significantdifferences(P<0.01).③Rats serum TNF-a level: Compared with the normal control group,model group rats serum TNF-a level are increased significantly(P<0.01). Compared withthe model group, the TNF-a level of Fuxin Tang low dose group decreased (P<0.05); theTNF-a level of Fuxin Tang high dose group, and captopril group are significantlydecreased(P<0.01).④Pathological changes of myocardium in rats: Normal control grouprats’ myocardial cell morphology, cytoplasmic, interstitial and horizontal (vertical) linesare normal. Compared with the model group, the myocardial tissue of rats are severelydamaged, the nuclei arranges irregularly, part of the myocardial nuclear are pyknosis, andfragmentation, muscle fibers are derangement, and interstitial edema varies, change likecardiomyopathy presenting. Compared with the model group, the treatments group ratmyocardial tissue morphology improve of different degrees.⑤The expression of Ratmyocardial protein Caspase-8and Bcl-2: Compared with the normal control group, theexpression of model group rats myocardial protein Caspase-8are significantly elevated(P<0.01), the expression of protein Bcl-2are significantly decreased (P<0.01). Comparedwith the model group,the expression of Fuxin Tang low dose group rats myocardial proteinCaspase-8, Bcl-2has no obvious statistical difference (P>0.05); Fuxin Tang high dosegroup rats myocardial protein Caspase-8expression are significantly decreased (P<0.01),protein Bcl-2expression increase significantly(P<0.01); captopril group rats expression ofmyocardial protein Caspase-8decreased significantly(P<0.01), protein Bcl-2expressionincrease significantly(P<0.05).2. Clinical research results:①The plasma BNP levels: Atthe end of the treatment, compared with the control group,the plasma BNP level of patientsin the treatment group was significantly lower (P<0.05), with statistical significance.②The patients life quality: At the end of the treatment, compared with the controlgroup,the Minnesota heart failure quality of life score of the treatment group patients aredecreased significantly (P<0.05),with statistical significance. Conclusion:1.It is effect that rat model of congestive heart failure are established byAdriamycin according to its cardiac toxicity.2.Fuxin Tang can improve the heart function of heart failure rats.3. Fuxin Tang can reduce the level of serum TNF-a in heart failure rats.4. Fuxin Tang can improve heart failure rats myocardial tissue morphology.5. Fuxin Tang can regulate the expressionof heart failure rats myocardial proteinCaspase-8, Bcl-2, thereby inhibiting myocardial apoptosis, delaying failure process.6. In the aspects of improving heart failure rats symptoms, regulating heart failure ratsmyocardial apoptosis and others, high dose of Fuxin Tang effects more significantly.7. In treating chronic heart failure, Western medicine in combination with Fuxin Tangcurative effects better than single western medicine treatment.
