Implant denture is a breakthrough in the field of Prosthodontics. It can significantlyimprove the patient’s chewing function,and can treat many difficult clinical cases whichthe conventional denture could not solve. Common complication of peri-implantitis isthe main reason of the implant denture repair failure. The plaque is the initiating factorof periodontitis. Implant structure is similar to the natural teeth surrounding structures,but has its own unique character. There is no periodontal ligament surrounding implantbody, but osseointegration interface between the implant and the alveolar bone. Thebacteriological characteristics of the peri-implantitis are not clear. Few reports wereused on quantitative research of implant bacteria, and the research was mostly limited toqualitative observation. Culture method for the quantitative detection of Implantbacteria and quantitative PCR detection were not used for the rapid detection of defectsin the clinical examination, because the examination cycle was long, testing conditionwas difficulty, and detection cost was high. Previous study of the electrochemicalmeasuring bacteria on the Implant state was used in this study to detect the gingivalcrevicular fluid P.gingivalis and S.sanguinis in quantitative determination. Through thestudy of two representative bacteria, we analyzed the level changes of beneficialbacteria and harmful bacteria around the implant. The analysis revealed more objectivefrom the microscopic point, and further revealed the mechanism of Peri-implantinflammation.It’s better to timely monitor the condition of the peri-implantinflammation, effectively control of the Peri-implant inflammation, and improve thecultivation of repair success rate.Thirty-six implants from thirty patients with more than one year treatment weredivided into three groups:(1) Thirteen implants from ten patients were selected inHealth-implant group;(2) Eleven implants from ten patients were selected inPeri-mucositis group;(3) Twelve implants from ten patients were selected inPeri-implantitis group. Select each patient in the mouth healthy or relatively healthy first molar as the control teeth; Electrochemical method was used to detect the gingivalcrevicular fluid P.gingivalis and S.sanguinis in quantitative determination.Also recordedthe corresponding stage of peri-implant clinical parameters including PPD, mBI,mPLI and BL.The results showed that:(1) peri-implantitis group of P.gingivalis detected higherthan the peri-mucositis group, peri-mucositis group of P.gingivalis detected was higherthan the Health-implant group; at the same time, peri-implantitis group of S.sanguinisdetected lower than Peri-mucositis group, Peri-mucositis group of S.sanguinis detectedlower than the Health-implant group, differences were statistically significant (P<0.05).(2)In Health-implant group, the implant and the natural control of the detection of theP.gingivalis and S.sanguinis in GCF compared with no significant difference (P>0.05);In Peri-mucositis group and the peri-implantitis group, the implant and natural controlof the detection of the P.gingivalis and S.sanguinis in GCF compared to all the obviousdifferences (P<0.05);(3) Three groups, differences in various clinical indicators weresignificantly (P<0.05);(4) Pearson simple correlation analysis, the detected of theP.gingivalis in GCF and clinical indicators of the PPD, mBI, mPLI and BL showed asignificant positive correlated, the detected of the S.sanguinis in GCF and clinicalindicators of the PPD, mBI, mPLI and BL has a significant negative correlation.The findings, to draw the following preliminary conclusions:(1) P.gingivalis and S.sanguinis in GCF is closely related to peri-implant health, In GCF,with the aggravation of the inflammation the P.gingivalis increase and the S.sanguinisreduce.(2) The detected of the P.gingivalis in GCF and clinical indicators of thePPD,mBI,mPLI and BL showed a significant positive correlated, the detected of theS.sanguinis in GCF and clinical indicators of the PPD, mBI, mPLI and BL has asignificant negative correlation. |
