Objective:This study is to investigate the differences of the clinical efficacy and the reasons of the LARS artificial ligament and four shares of the body semitendinosus and gracilis tendon reconstrction of the anterior cruciate ligament after the knee injury.Methods:Retrospective summary and analysis of the clinical results of LARS ligaments and autogenous semitendinosus and gracilis tendon in ACL reconstruction on38patients from2010.10to2012.04in Shandong University Qilu Hospital orthopedic. LARS ligament group (18cases), autogenous semitendinosus and gracilis tendon20cases.Through postoperative Lachman test, the IKDC score and the Lysholm score of two groups patients,we will compare the differences between two methods of the anterior cruciate ligament reconstruction surgery.Results:In Lachman test, both groups have no significant difference (P>0.05) and there were not statistically significant before surgery; after six months of operation, the two groups compared, P<0.05, statistically significant; after1yearthe two groups compared, P>0.05, not statistically significant. In the IKDC score, preoperative ST/G group IKDC score mean52.05±5.09, median51.5(46~62), with LARS group mean of49.50±5.06, median value of48.5(41~57); after six months of operation the ST/G group were77.20±2.84, median77(73~82), with LARS group mean of79.00±2.33, median79(75to84); after one year of operation the ST/G group were89.35±2.01, median89.5 (86-93), with LARS group mean of89.89±3.22, median value of90(84-95). By rank-sum test, before surgery, six months after surgery after surgery and one year after operation between the two groups, P>0.5, no significant difference. Lysholm score before surgery, the ST/G group mean48.60±4.04, median48.50(42-55), with LARS group mean of48.06±6.18, median47.00(40-60);6months after surgery the ST/G group mean of79.85±.31, median80.00(74~86), with LARS group mean of80.06±3.59, median80.00(75~86);1year after surgery the ST/G group mean of88.30±2.76, median88.00(84~93), with LARS group mean of90.00±2.61, median90.00(84to94).By rank sum test, before surgery、6months after surgery, one year after surgery between the two groups, P>0.5, no significant difference.This indicats that, with an autogenous tendon and LARS ligament ACL reconstruction,,there was no significant difference in the knee function recovery、the clinical efficacy and patient satisfaction.Conclusion:After6months and1year of the operation,there was not significant difference on the IKDC and the Lysholm knee score about the tendon and the LARS ligament reconstruction of the anterior cruciate ligament.It means that there was not significant difference between the two kinds of reconstruction of the ACL in the restoration of joint function the same clinical efficacy. The LARS ligament group was better than the autologous tendon group in Lachman test after six months afteroeration, there was no significant difference after1year. It means that the LARS ligament reconstruction can make the knee stable in a short time but with autologous tendon are slower, however,with the autologous tissue ingrowth, and the difference will disappear. ACL reconstruction surgery by LARS ligament costs less time and patient injury,can get quicker recovery, certainty of clinical efficacy, no significant complications,so it is worthy of promotion.
