ObjectiveThe investigate on the protective effect of chuan bei pi pa gao and yun bei yao in the oral mucosa radiation injury of head and neck carcinoma.Methods60patients of head and neck carcinoma with Radiation therapy were randomly divided into treatment group and control group. Patients in the treatment group received regular oral nursing during irradiation course, moreover, take orally chuan bei pi pa gao and yun bei yao since patients in grade Ⅱ mucosal response, while receiving conventional symptomatic treatment. Patients in the control group received daily regular oral nursing and the conventional symptomatic treatment; moreover, intravenous infusion of drug hormones and antibiotics, and even intravenous nutritional support for patients in grade Ⅲ/Ⅳ mucosal response.ResultsOropharyngeal mucositis was evaluated according to the protocol proposed by RTOG. Mucosal injury at stage Ⅰ, Ⅱ, Ⅲ and Ⅳ in the two groups was occupied10%,60%,30%,0and0,6.67%,33%,43.33%,16.67%, respectively, and there was also significantly difference between the two groups (P<0.05). The cases in treatment group and control group who were observed Ⅰ-grade pain, Ⅱ-grade pain and Ⅲ-grade pain occupied10%,80%,6.67%and6.67%,66.67%,20%respectively, and there was significantly difference between the two groups (P<0.05). With increasing radiation dose, the pain and oral mucosa radiation injury situation of two groups of patients were different and also showed a significant difference (P<0.001).With increasing radiation dose, both groups appear to the oral mucosa damage and he degree of damage gradually aggravate. When the does reach DT30Gy、DT40Gy、DT50Gy. DT60Gy and DT70Gy, the conditions of two groups of Radioactive mucosal inflammation are compared with rank-sum test, P<0.001, The difference between the two groups were statistically significant, degree of injury of the observation group are lighter than the control group. Radioactive pain always aggravates with the Radiation dose. When they reach DT30Gy、DT40Gy、DT50Gy、DT60Gy and DT70Gy, the conditions of two groups of pain are compared with rank-sum test, P<0.001, The difference between the two groups were statistically significant, degree of injury of treatment group in are lighter than control group. When compared with the weight changes of the two groups of patients who received radiotherapy to the seventh week.13cases of the treatment group maintain weight, and17cases lose weight. However, in the control group,6cases maintain weight, and24lose weight. The difference between the two groups is statistically significant,(χ2=3.824,P=0.050).When compared with the situation of pharyngeal swab culture of the two groups of patients who receive radiotherapy to the seventh week,11cases of positive result were found in treatment group, and19cases with negative result. Control group has15cases with positive result, and4cases with negative result. The difference between the two groups were statistically significant, χ2=1.086, P=0.297)ConclusionChuan bei pi pa gao and yun bei yao can decrease the degree of radiotherapy adverse reactions and relieve the symptoms of acute radiation-induced oral mucositis in patients with head and neck carcinoma receiving radiotherapy.
