Research BackgroundThe new rural cooperative medical care system was set up in Shandong Province in2003. Since its foundation the system has experienced a rapid development with the pilots extended and the coverage expanded. The capability to raise money has been upgraded and compensation mode has been improved. Meanwhile the fund is used in a more rational way and the utilization ratio of the medical service is largely enhanced. The payment mode has been reformed and instant bill closing has been realized too. The pilot sites for medical security in terms of major diseases have been expanded and it gradually made it easier, both operationally and financially, for farmers to access to medical treatment. Our work is to evaluate the new rural cooperative medical care system comprehensively and subjectively and provide the foundation for its further development.Research ObjectiveAnalyze the operation conditions of the new system between2003and2010and evaluate the overall performance of the system between2008and2010. Suggestions should be conducted by identifying problems and analyzing the influences.Data and MethodologyThe data information between2003and2010was collected and inputted into database. The SPSS V19software and fourteen indexes have been used in measuring the performance of the system, such as the participation rate, the coverage rate for impoverished people, fund raised per capita, funding rate, funding proportion, benefit rate, compensation rate, fund expenditure inclination and medical treatment inclination, etc,. Rapid assessment has been made in accordance with the ten indexes launched by Ministry of Public Health, analyzing the overall operations in all cities and regions in Shandong Province before ranking the cities according to their scores either in single index or in comprehensive indexes.Major Achievements1. The carrying out of new rural cooperative medical care system has experienced great development from pilot experiments in the beginning to wide promotion at present. The participation rate for farmers has been enhanced year by year, from82.07%in2003to99.6%in2010. Fund raising level per capita has turned over several times, rising from20RMB to200RMB per capita in2011, which is9times more than the initial level. The fund utilization rate has also been improved, rising from69.12%in2003to99.42%in2010. Hospitalization benefit rate has also been enhanced, rising from2.54%in2005to7.77%in2010. The compensation rate for hospitalized people has been on the upward trend, rising from20.72%in2003to40.41%in2010. The farmers who participated in the cooperative system enjoy a more convenient medical service and the regions covered in the new medical care system have made real instant bill closing. The new system evoked warm responses among farmers because it is more convenient and easier for farmers to receive medical treatment.2. In undertaking performance evaluation for the new cooperative medical care system, the evaluation indexes and methods have been analyzed in combination with the data information of the participation rate, fund raising amount per capita, hospitalization benefit rate, compensation rate, etc. Conclusions and SuggestionsThough the new rural cooperative medical care work has made certain achievements, there are some problems rendered in the development, such as:the lack of equality in the city-based unified compensation policy, the imbalance between funding raising amount per capita and sub-average medical consumption amount, weak management in the new cooperative system, over flowing of medical service fund on regions outside the county, the insufficiency of related laws and regulations as well as method payment reformation, the lagging back of information construction in the provincial departments.This thesis has also made the following suggestions for the new rural cooperative medical care work in Shandong Province:develop a sustained fund-raising regime, push forward the integration of outpatient service, promote the pilot sites for medical security in terms of major disease, strengthen the management for the new rural cooperative medical care service and underline the supervision for medical institutions and expenses as well as the supervision for the foundation.
