【Objective】To observe the changes of blood glucose,insulin and Glucagon-likepeptide-1(GLP-1) after Ileal transposition(IT) surgery in Goto-Kakizaki (GK) rats.Toinvestigate the hypoglycemic effect and explore possible mechanisms involved.【Methods】Twenty male GK rats were randomized into two groups: the first groupundergoing a sham operation (C group) and the second group undergoing IT surgery(D group). Twenty male SD rats were randomized into two groups: the first groupundergoing a sham operation (A group) and the second group undergoing IT surgery(B group). Fasting plasma glucose (FPG), oral glucose tolerance(OGTT), plasmalevels of insulin and glucagon-like peptide-1level (GLP-1) were determined. Detectthe level of GLP-1expression with immunohistochemical methods. Detect the levelof GLP-1R mRNA expression by RT-PCR.【Results】1. FPGCompared with the preoperative, the fasting blood glucose of D group rats postoperation has begun to decline after1week,2weeks post operation the fasting bloodglucose from8.25±0.52mmol/L decreased to6.62±0.54(P<0.001)mmol/L (P<0.001),8weeks post operation the fasting blood glucose decreased to4.78±0.48mmol/L (P<0.05), and keep the low level for weeks,the fasting blood glucose of D group wassignificantly lower than the C group post operation (P<0.05), the fasting bloodglucose of A and B group has no significant changes post operation.2.OGTTCompared with the preoperative, the OGTT of D group has changed significantsince post operation2weeks.C group has no significant changes post operation. 3.InsulinCompared with group A and B,group C and D have significant difference beforeand after surgery in all the operative groups through the entire follow-upperiod(P<0.05). Campared with pre-operation, the insulin level of D group wassignificant change (P<0.05).4. GLP-1Compared with the preoperative, the GLP-1of D group1week post operationhas begun to rise significantly, the GLP-1from6.56±0.90pmol/L increased to12.35±1.43pmo1/L (P<0.05).4weeks post operation, increased to the26.1±2.64pmol/L (P<0.05). The GLP-1of D group was significantly higher than C group andpreoperative. The GLP-1of B group was significantly higher than A group andpreoperative.5. IHCA group ileal mucosal tissue GLP-1had no expression. B group ileal mucosaltissue GLP-1expression was positive, showed yellow, positive rate20%.C group ilealmucosal tissue GLP-1had no expression.D group expressed high,showed brown,positive rate100%.6.RT-PCRGroup D of GLP-1R mRNA expression was significantly higher thanothers(P<0.001).【Conclusion】1.IT surgery on type Ⅱ diabetic rats has a hypoglycemic effect.2.IT surgery makes the food fast access to the ileum, to stimulate the mucosalcells, GLP-1secretion increased, leading to insulin secretion increased, which playsan important role in the blood glucose lowering mechanism.
