With the development of scientific and medical, percutaneous vertebral angioplasty (percutaneous vertebroplast PVP) and percutaneous vertebral kyphosis angioplasty (percutaneous kyphoplasty PKP) emerged as a new type of minimally invasive surgery, they remission in patients withpain, strengthening the vertebral body, to maintain spinal stability play a good role. But the kinds of literature reported different experimental results of PVP and PKP, clinical, osteoporosis, vertebral compression fractures (osteoporosis vertebral compression fractures OVCF) of elderly patients, the current lack of valid evidence to prove which a surgical approach is safe and effective.Objective:By comparison of PVP and PKP surgery before and after the VAS score, the anterior vertebral, the central height restoration and vertebral kyphosis Cobb angle changes, intraoperative injection of bone cement volume and follow-up bone cement leakage, adjacent level fracturesthe rate of complications such as observation, evaluation of their treatment of osteoporotic vertebral fractures in elderly patients with mid-term clinical efficacy and safety.Subjects and Methods:Met the inclusion criteria of osteoporotic vertebral compression fractures in elderly patients (≥65years)73cases, according to the patient’s own wishes, the38cases treated with PVP and PKP treatment in35cases. On the preoperative and postoperative day3,1year,2years VAS score. At the same time on two kinds of surgical postoperative period the anterior vertebral height and Cobb angle changes of central and two kinds of postoperative bone cement leakage and during follow-up near the vertebral fracture rate for statistical analysis.Results:1.A11patients were followed up for2years, significantly alleviate postoperative pain, preoperative and postoperative VAS scores between the two groups no statistical difference (p>0.05).2. Differences in the amount of cement injected in two groups of patients significant (p<0.05).3. PKP group immediately after spinal kyphosis Cobb angle was significantly less than before surgery, the gap has a significant (p<0.05) after1year and2-year follow-up, PKP slightly worse in the spinal kyphosis Cobb angle correctioni mmediately after surgery, but still better than the PVP (p<0.05).4. PVP could partially restore vertebral height (p<0.05); PKP almost wholly to restore vertebral height (p<0.05); PKP than PVP more to restore vertebral height (p<0.05).5. PVP and PKP difference in the incidence of complications of bone cement leakage andClose to the incidence of vertebral fractures was not statistically significant (p>0.05).Conclusions:(1)Two minimally invasive surgery, postoperative and follow-up terminal can significantly relieve the pain, the difference was not statistically significant.(2) Two minimally invasive surgery, postoperative elevation of injured vertebral height compared surgery were statistically significant, PVP and PKP can raise the height of the fractured vertebral.(3) Two methods of minimally invasive surgery in patients with anterior vertebral height and the central height difference was statistically significant, PKP group than in the PVP group exist in the recovery of compressed vertebral height advantage.(4) The quantity of two methods of minimally invasive surgery in the intraoperative injection of bone cement were significantly different, less PVP on the number of injection of bone cement has an advantage.(5) The two methods of minimally invasive surgery, statistically significant differences in postoperative kyphosis Cobb angle recovery. PKP group than the PVP group, the recovery of Cobb angle, kyphosis advantage.(6) PVP and PKP difference in the incidence of complications of bone cement leakage andClose to the incidence of vertebral fractures was not statistically significant.
