Purpose:Discuss the understanding of subacute thyroiditis of traditional Chinese medicine (TCM) by using prescription of clearing heat and shrinking thyroid. To observe the therapeutic efficacy brought by this prescription in treating the syndrome of stagnated heat of liver channel of subacute thyroiditis and to investigate the mechanism.Methods:A randomized, controlled trial was conducted on50patients with the syndrome of stagnated heat of liver channel of subacute thyroiditis classified into two groups. The patients of control group (n=25) were treated by western medicine while the patients of treated groups (n=25) were treated by western and Chinese medicine. Three months was a course of treatment. The observed indexes included both change in the clinical symptoms and signs, index about thyroid hormone、ESR、3H thyroid iodine uptake rate of131I and harmful reaction after using glucocorticoid. All data were analyzed by the SPSS15.0statistics software.Results:Before treatment, the gender, age, course of disease of the two groups had no differeces(P>0.05).①There is a significant difference between the two groups in the total and TCM curative effect. The total curative effect of treatment group was96.00%, the TCM curative effect of treatment group was96.00%, while total curative effect of control group was80.00%, the TCM curative effect of control group was52.00%. The treatment group gained an advantage over the control group in improving TCM symptoms and total curative effect (P<0.05).②The FT3,、FT4、ESR of the two groups were both decreased, the TSH of them were elevated, compared with pre-treatment there is statistically significant (P<0.01). The improvement of TSH、FT3、FT4of treatment group has a remarkable statistically significant (P<0.01), compared with control group. And the treatment group had an advantage over the control group in FT4(P<0.05).③The3H thyroid131iodine uptake of the two groups were elevated after treatment, compared with pre-treatment, there is statistically significant (P<0.01). And the treatment group had an advantage over the control group(P<0.01).④Compared to the control group with side effects connected to glucocorticoid, there were three patients in the treatment group who have suffered side effects connected to glucocorticoid. The side effects rate was12.00%. There were fifteen patients in the control group who have suffered side effects connected to glucocorticoid. The side effects rate of control group was60.00%. And the difference between the two groups in side effects connected to glucocorticoid is significant(P<0.01).⑤Safety indicators:Liver and kidney function tests were in the normal range. Both groups were no severe adverse reactions. Conclusion:From the preliminary clinical research, prescription of clearing heat and shrinking thyroid combined with western medicine can not only improve the clinical symptoms of TCM, but also can improve thyroid function reduce ESR, improve life quality. The method is safe and effective. It is better than using western medicine only and has a broad prospect of application..
