Objective: To investigate the expression and clinical significance of solubleB7-H4(sB7-H4)in the serum in patients with non-small cell lung cancer(NSCLC), lookingfor early diagnosis of NSCLC immunological parameters. Methods: we used ELISA kit toanalyze the levels of sB7-H4in51serum samples from patients with NSCLC and48samples from benign lung diseases(BLD), and analyze its relationship withclinicopathological parameters of NSCLC, and study the pre-operation and post-operationchange of sB7-H4. Meanwhile, serum CEA, CA125, CYFRA21-1were detected byelectrochemiluminescence assay,and explore the clinical value of combined determinationof sB7-H4, CEA, CA125, CYFRA21-1levels in serum for diagnosis in NSCLC. Results:⑴s B7-H4serum levels in NSCLC(49.78±8.78) μg/L was significantly higher than benignlung disease(27.34±9.39)μg/L,(t=12.288,P<0.001). serum level of sB7-H4inpost-operation was lower than that in pre-operation,(t=6.330,P<0.001).⑵The level ofsB7-H4expression in serum with good differentiation or moderate differentiation and poordifferentiation was (44.03±8.82)μg/L,(53.63±8.46)μg/L,(62.48±8.27)μg/L, the sB7-H4protein expression in serum with poor differentiation was more higher than that with gooddifferentiation or moderate differentiation,(t=4.298, P<0.001; t=3.301, P=0.004).⑶Theexpression of B7-H4in lymph node matastasis group (53.59±8.97)μg/L is higher than thatin the no lymph node matastasis group(44.76±8.63)μg/L, the difference has statisticalsignificance(t=3.536, P<0.001).⑷The serum level of the expression of B7-H4inpathological stagingⅠ was lower than that inpathological staging Ⅱa ndpathologicalstaging Ⅲ (t=4.080, P<0.001; t=4.603, P<0.001).⑸In single detection, the sensitivityand youden’s index of sB7-H4(74.5%,0.537) was superior than those of CEA,CA125,CYFRA21-1, and combined detection can improve the sensitivity(90.2%). Conclusion: sB7-H4highly express in NSCLC, could promote the occurrence of lungcancer and immune escape, and is a new immunological indexes. It can singnificantlyimprove the sensitivity of the diagnosis of NSCLC with sB7-H4and other markers.
