Objective:The development of recurrent Acute exacerbation of chronic obstructive pulmonary disease (AECOPD) is important risk factors that lead to the progress of chronic obstructive pulmonary disease (COPD), and one of the main reasons for patients with medical treatment and death. Traditional Chinese Medicine (TCM) treatment of AECOPD have a certain effect and advantages, However, it needs more further studies on mechanism. Airway inflammation, protease/anti-protease imbalance has become a focus in pathogenesis of COPD. Based on previous studies, This study is to investigate clinical therapeutic effete of Patients with AECOPD of tanreyongfei and biaoxie syndrome by treatment of TCM preparation (Kechuan decoction) combined with Western medicine, and the pathogenesis of treatment of TCM, their cellular and molecular mechanisms, and how these underlie physiologic abnormalities and symptoms characteristic of AECOPD.Methods:According to the related diagnosis standard, Collected80cases diagnosed in patients with AECOPD, we assigned40to the control group and40to the experimental group at random. The control group were treated by Conventional western medicine, At the same time, the experimental group was dosed kechuan decoction additionally, lasted12days. To compare the result of two groups by analyzing clinical symptom and sign, blood gas analysis, pulmonary function, the level of MMP-9, TIMP-1、IL-10in peripheral blood of patients were assessed by ELISA.Results1、The overalleffective treatment of experimental group were94.74%, control group were91.89%, there were significant difference (P<0.05).After the treatment, symptoms up to the effective time of the experimental group apparent less than control group, there were significant difference (P<0.05).2、Experimental group after the treatment in Cough, cough up phlegm, bosom frowsty, gasping for breath apparent reduce control, its differences were statistically significant (P<0.05). clinical symptom and sign in both groups have significant difference.3、After the treatment, The Kechuan Decoction can help to improve the indexes of blood gas analysis, and the experimental group is superior to the control group.4、MMP-9、TIMP-land IL-10in the peripheral blood of both groups reduce, showing significant difference, we can see that the effect of the experimental group on reducing MMP-9. TIMP-1、IL-10in the peripheral blood is more significant than the one of the control group5、Safety evaluation:treatment by syndromes by syndromes differentiation of TCM is safe and effective,no severe adverse reaction reported. Only a few reporting adverse event,the most frequent of which were discomfort of the stomach uand diarrhea Conclusion:Kechuan decoction combination with routine western medicine reduced syndrome, improved anoxia and carbon dioxide retention and debased the level of IL-10, MMP-9/TIMP-1relative to group by routine western medicine in clinical trials on patients with AECOPD.
