Purpose:To study the clinical efficacy and potential mechanisms of JI SHENG SHENQI WAN administrated in elderly patients with acute renal failure (ARF).Methods:According to the diagnostic criteria of ARF,30eligible elderly patientsfrom ICU of Shandong Qianfoshan Hospital were randomly assigned into2groups bymeans of random number table. Patients in both groups received comprehensive treatmentof western medicine. One group (treatment group) additionally received JI SHENG SHENQI WAN orally once per day, while the other group (control group) received normal salineof the same dose and frequency.7days is defines as a course of treatment, our observationperiod is14days. We performed TCM (traditional Chinese medicine) syndrome scoringand APACHE II scoring before and after the above different treatments of both groups, tocalculate and compare the treatment efficacy. We also monitored the serum creatinine,urine volume, and perimeter of leg at the cross section of10cm above the superior marginsof patella at day0, day7, and day14, to observe the incidence of multiple organ failure(MOF) and uremia, and mortality on day28.Results:TCM syndrome scoring and APACHE II scoring before treatment showed nosignificant differences (P>0.05), demonstrating similar severity of2groups. Symptoms ofall patients improved after treatments. TCM syndrome scores decreased in both groups (P<0.05), which is more significant in the treatment group(P<0.05). The urine volume, andperimeter of leg at the cross section of10cm above the superior margins of patella ofpatients in treatment group at day7were significant different with those at day0(P<0.05).The serum creatinine, blood urea nitrogen, and renal failure index of patients in treatmentgroup at day14were significant different with those at day0(P<0.05). After treatment,the incidence of MOF and uremia were significant lower in treatment group than in control groups (P<0.05). The mortalities showed no statistical difference.Conclusion:JI SHENG SHEN QI WAN can significantly relieve oliguria and edema,improve clinical response rate, decrease clinical incidence of MOF and uremia, but can notdecrease mortality at day28.
