BackgroundThe establishment of Essential drug policy is one of the key programs for health system reform and it is also a Civil Engineering which has profound meaning for Chinese people. With the implementation of this policy, the whole drug using system has changed, including a series of operation mechanism like bidding, purchasing, dispatching, subsiding and supervising. Under the new system oriented by Essential Drug Policy, every stakeholders are trying their best to explore new strategies to adapt the new environment and meanwhile seeking new chances to broaden their profit. It is crucial for Chinese government to establish a harmonious environment in which all the stakeholders will complete and perform in the direction we need.ObjectivesThe study aims to explore the new relationship between stakeholders under the implementation of Essential Drug Policy. Through mapping the stakeholder circle, we tried to figure out the potential hazard and unused power within the new essential drug system. Evolutionary game theory has been used in this study to deal with benefits between different stakeholders. It produced a fundamental theory basis for Chinese government to create a harmonious operation mechanism by guiding stakeholders to form cooperative games.MethodsIn this study, Method of literature was used to collect papers related to essential drug policy home and broad, questionnaire survey method and Delphi method were used to collect quantitative data. The study used self-designed questionnaire<Detection Indicators for Essential Drug Policy> to survey31provinces In China. There were about20health clinics in towns and townships which submitted questionnaire in each province. The Delphi experts group was formed with36experts and26of whom passed the authority examine. Model construction method has been used to explore the characteristic of stakeholders in the new essential drug system. This study used descriptive analysis to express statement and SPSS17.0software to do the calculation of critical indicators.Main Results1. The implementation of essential drug policy is going smoothly. Central region of China has the lowest prescription price and it’s range of price decrease from5%-15%. Hei Longjiang Province has the largest scope of price decrease which is46%. Price decrease of provinces in western region of China are range from10%-20%and most of them are above the national average.2. From picture one we could find that central government of China was dominant stakeholder of the whole stakeholder model. Provincial government was definitive stakeholder, media and drug store were dormant stakeholders, pharmaceutical enterprises and Distribution enterprises were dangerous stakeholders, local government and medical institution were dependent stakeholders, patients, community residents and medical insurance were discretionary stakeholders.3. From picture one we could find the preconditions of forming a cooperative game system, there were:1) The benefits of cooperation exceeded the unilateral speculative benefits (Q-v-d>0);2) The possibility of forming a cooperative game system would increase with the rising of cooperation benefits, reducing of cost and restricting of unilateral speculative benefits.Countermeasures and SuggestionsAccording to the results, we offer the following proposals:pharmaceutical enterprises and Distribution enterprises were dangerous stakeholders for essential drug policy. Because of their potential threat to the implementation of the policy, we should motivate them to form cooperative game system on their own will. Specific measure is to increase the weight of technical standard during bidding and explore more scientific and reasonable method for bidding model. Medical institutions are functional units close to the core of essential drug policy and in the position of front line for using essential drugs; however, their lack of motivation for using essential drugs will finally harm the implementation. Chinese government should issue policy to regulate the use of essential drugs while reverse the role of medical institutions from economic units to social service units. |
