Analysis The Clinic Features In347Cases Of Children With Viral Myocarditis

Objective:To analysis the clinic and assistant examinations in children with viral myocarditis(VMC),in order to sum up the experience of diagnosis and therapies.Methods:To retrospectively study symptoms,signs, etiology, cardiac creatine, troponin, electrocardiogram(ECG), transthoracic echocardiography(TTE), chest X-ray, Holter of patients with VMC inpatient in Children’s Hospital of Chongqing Medical University from January2008to January2010Results:There were347VMC patients involved,217males,130females,their age from1month1day to15years7months(average3.47±4.03year old). Prodromal infection318(respiratory tract201, digestive tract26,both91).The clinical manifestation from asymptomatic to heart failure, Adams-Stokes syndrome,but most common manifestation are irritable, precordial discomfort,decreaseing of activity level,signs are weak cardiechema, arrhythmia. Abnormalities of assistant examinations are:ECG310/333(93.1%),cardic creatine304/344(88.4%),TTE242/295(82.0%),holter89/121(73.5%),cardic troponin118/315(37.5). All the patients’therapy is heteropathy and comprehensive therapy, meanwhile great dose intravenous glucocorticoid was used to the severe cases.And one of patientsthem present atrioventricular conduction block (AVB) and Adams-Stokes syndrome,and was embedded pace-maker.There was no death in all patients(healing104, improve243)Conclusion:Most patient withof VMC had prodromal infection before onset. Although some patient onset and devolp rapidly,if had timely diagnosis and treatment the prognosis was still weill. Even though the clinical manifestations are atypical,cardic physical examination carefully can findshows the abnormal sign such as weak cardiechema,arrhythmia,tachycardic and so on.ECG, cardiac creatine,TTE are very important to the diagnosis,Holter can catch sight of transient arrhythmia, injure of autonomic nerve. To those severe patient great dose intravenous glucocorticoid should be used in time,when Adams-Stokes syndrome or Ⅲ°AVB happened, pace-makers should be embedded as early as possible.
