ObjectiveTo study the relationship between anxiety or depression and condition in patients with chronic obstructive pulmonary disease. To investigate the affect of anxiety and depressive on the state of the whole body in patients with COPD. Understanding of COPD complicated with anxiety and depression of the common syndromes of Traditional Chinese Medicine. To offer guidance for their comprehensive clinical treatment.MethodsOne hundred and twenty three COPD stable patients which stage for3-4level inpatients or outpatients case from Guangdong Province Traditional Chinese Medical Hospital during February2011to April2012were enrolled in this study. The Self-rating Anxiety Scale(SAS) and Self-rating Depression Scale(SDS) were investigated to collect the information of patients.123cases with COPD patients were divided into non-anxiety group, mild anxiety group, moderate to severe anxiety group or non-depression group, mild depression group, moderate and severe depression group which according to the results of SAS and SDS. Statistical analysis the gender, age, course of cough, course of shortness of breath, symptom score of TCM, BODE, pulmonary function between groups. The possible influencing factors about the anxiety and depression in the inpatients with COPD were analyzed by adopting multi-factor Logistic regression analysis. Investigation of COPD complicated with anxiety and depression of the common syndromes of traditional.ResultsIn123cases with COPD patients,75cases were not in the anxiety state, accounting for60.98%while48cases in anxiety state, accounting for39.02%. The incidence rate of anxiety with33mild,15moderate to severe anxiety cases, accounting for26.83%and12.20%respectively. In COPD patients,66cases were not in the depression state, accounting for53.66%while57cases in depression state, accounting for46.34%. The incidence rate of depression with35mild,22moderate to severe depression cases, accounting for28.46%and17.89%respectively. According to the result of SAS and SDS, there were significant differences in the indexes of course of cough, course of shortness of breath, pulmonary function, symptom score of TCM, BODE,6MWD, BMI between the groups, who were anxiety or depression and non-anxiety or non-depression(P<0.05). No statistical meaning exists as the result of gender and age between the groups (P>0.05). Regarding course of cough, course of shortness of breath, symptom score of TCM, BODE of five indexes as the independent variable, and regarding anxiety or depressive state as the dependent variable, multi-factor Logistic regression analysis suggested that symptom score of TCM was the influencing factor of COPD comorbid anxiety or depression. The main syndrome of COPD with anxiety or depression patients is lung and spleen qi deficiency, phlegm and blood stasis obstructing lung, And in COPD complicated with anxiety in33.33%of patients with liver-qi stagnation syndrome,20.83%with heart lienal two deficiency; during the complicated with depression in35.09%of patients with liver-qi stagnation syndrome,21.05%with heart lienal two deficiency.Conclusion48among123cases with COPD inpatients suffer from anxiety state, accounting for39.02%, and57among123cases with COPD inpatients suffer from depression state, accounting for46.34%. This suggests concomitant high incidence of depression in COPD patients. It is more likely suffered from anxiety and depression when to extend the course of cough or shortness of breath, pulmonary function became poor, decreased the6MWD and BMI, MMRC grade elevation, increased symptom score of TCM in COPD patients. Anxiety and depression can affect the state of the whole body in patients with chronic obstructive pulmonary disease. During the treatment of the COPD combined with anxiety or depression patients,we should pay attention to the symptoms and psychological.
