OBJECTIVE:To investigate the relationship between emotional change and epigastric pain, the diagnostic characteristics and patient distribution of Emotional factors induced Epigastric Pain (abbreviated as EEP below) in order to provide a reference for diagnosis and treatment.METHODOLOGY:Based on ancient and recent studies combining with clinical research methods, collect375cases of epigastric pain in Hong Kong,217cases of which are EEP. With the use of SDS, SAS survey forms to determine the emotional factors involved. With the use of TCM epigastric pain survey form to obtain patient related information and their symptoms and doctor diagnosis.RESULTS:1. Of the375cases of epigastric pain,57.87%of them are EEP.81.55%of the patients with EEP are female. Among the217cases of EEP,77.42are female and only22.58%are male.57.14%of the cases concentrated between the age group30-39.18.43%are of the age group50-59.24.42%are of the age groups below30and over60.2. Among the EEP cases,63.13%have SDS and26.87%have SAS. Male patients have77.55%of the SAS cases and female patients have75%of the SDS cases. 74.56%of the SDS cases are patients below39and over60years old. For the age group40-59, the SDS and SAS percentiles are much closer at52.38%and17.62%,47.5%and52.5%.3. The EEP cases are diagnosed as the following:Liver Affected Stomach Illness (LASI37.79%)> Liver and Stomach Congested Heat (LSCH21.2%)>Liver Congested Weak Spleen (LCWS19.82%)>Stomach Yin Weakness (SYW8.76%)> Poor Stomach Blood flow (PSB7.83%)>Spleen&Stomach Weakness (SSW4.61%). LASI, LSCH, LCWS are the three most common accounting for a total of78.8%.4. Among the EEP patients, LASI is the most common and accounts for30.61%male and39.88%female. The second highest for male is LSCH (28.57%) and LCWS (20.83%) for female. SSW is relatively rare, only accounts for2.04%for male and5.36%for female. For various age groups, LASI is the most common, being35.29%,47.54%,38.1%,27.5%and31.58%. SSW and PSB are relatively rare. As the age increase, cases of weakness tend to increase as well. The patient distribution being8.82%,9.84%,12.7%,17.5%and26.32%. The results above are of no statistical significance.5. EEP due to SDS leading to LASI, LCWS and LSCH are the most common. It accounts for81.75%of the cases. SAS induced LSCH, LAST and SYW is the most common. It accounts for76.25%. The results above do have statistical significance.6. Those with less than2years of EEP, LASI cases are most common accounting for70%and39.13%. Those with2-3years of EEP, LSCH cases are most common accounting for28.85%. Those with3or more years, LCWS cases are most common accounting for32.61%. SYW and PSB are often found among patients with2-3years of EEP. SSW are usually found among patients with over2years of EEP. The results above do have statistical significance.CONCLUSIONS:1. Epigastric pain in Hong Kong are often induced by emotional changes. It is the leading cause among female. It is especially common among youths and adults. Female and more susceptible than male.2. Epigastric Pain in Hong Kong due to SDS is more common than SAS. Male patients mainly have SAS whereas female patients mainly have SDS.3. EEP cases in Hong Kong have special characteristics and patient distribution. The most common type is LASI, after that is LSCH and LCWS. The least common is SSW. 4. Hong Kong EEP cases are not affected by gender and age.5.Hong Kong EEP cases are related to emotional factors. Depression causes congestion and poor blood blood. Anxiety causes heat and is damaging to yin.6. Hong Kong EEP cases are closely related to emotional factors. Most of the cases are short short. As the illness protract and progress, weakness, poor blood flow circulation and imbalances increase as well.
