Objective:To summarize the clinic-pathological features, explore comprehensive therapy, and analyze the prognosis and prognostic factor of metaplastic breast carcinoma (MBC).Methods:To analyze retrospectively the clinical data of all patients diagnosed with MBC between January2008and March2011and review the related literature.Results:There were twenty patients. The median age was57years. All patients presented breast mass and the average tumor diameter was3.0cm. All patients received surgery, and were confirmed as MBC by the pathological and immunohistochemical examinations. Fourteen cases were the pure epithelial type and6cases were the mixed epithelial/mesenchymal type. The negative ER-. PR-and Her2/neu receptors (the triple negative) were in twelve cases. Five patients were in stage1.13were stage II, and1was stage Ⅲ. Sixteen patients had undergone breast modified radical mastectomy, and postoperative chemotherapy and radiotherapy had been given to16and4patients. Follow-up time ranged from12.1to49.7months. Eighteen patients survived.2cases with spindle cell carcinoma and carcinosarcoma respectively died at10.2and29.8months after operation, and the2-year survival rate was95%. Five patients had the recurrence with a median follow-up of28.6months (range4.3-36.6months), and the2-year recurrence-free survival rate was88%.Conclusion:MBC is a rare breast malignancy, and always present breast mass without characteristic clinical manifestation. It is confirmed by the pathological and immunohistochemical examination. The pathological character is that the spindle cells, squamous cells and stromal elements are dominated in metaplastic carcinoma breast, and few cases of the spindle and squamous cells carcinoma are without adenoma tissue. The principle of treatment is similar to the invasive carcinoma. The prognosis is correlated with tumor size, pathologic type, hormone receptor and the staging. The prognosis of the epithelial type is better than that of the carcinosarcoma.
