Development And Application Of The Statistical Metadata Used In The Community-based Health Record

Objective：Among specifications which have been released by the Ministry of Health,two areas are still blank: which indicators should be included incommunity-based health record and how to describe statistical indicators in it.In order to standardize the construction of community-based health record ofelectronic health record for residents, this study is intended:(1) to develop theindicator framework to provide content reference for constructions ofcommunity-based health record in China;(2) to apply the indicator frameworkin the construction of community-based health record in Wuwei;(3)to build thestatistical metadata framework and (4) to develop instances of statisticalindicators described by the statistical metadata set.Method:The unified modeling language was used to describe the indicatorframework of community-based health record and to build statistical metadataset. The design tool is Rational Rose. Field survey method was used to analyze the applicability of community-based health records in Wuwei City; Protégétool was used to manage and describe the instances of statistical metadata.Results:1．The indicator framework of community-based health record containsidentifier information and four domains which are health status, non-medicaldeterminants factors, the performance of health systems and community andhealth system. Combined with the establishment of the paper health records andthe health infrastructure in Wuwei city, we built community-based health recordmodule of Wuwei City which is composed of4dimensions and96indicators.2．Field survey found that leaders’ attention and the set of indicators wererelated to the quality of records.3．In order to meet the needs in the data collection practice,42statisticalmetadata items were developed to form metadata set which was composed of4domains and9sub-domains. Furthermore, the metadata set was applied in thedescription of chronic disease.Conclusion:In this study, indicator framework of community-based health record basedon related national norms and practice was used in the construction ofcommunity-based health records in Wuwei City, which can provide contentreference for the flourished construction of community-based health record allover the country; instance of statistical indicator described by the statisticalmetadata items can provide useful experiences for improving the quality ofstatistical indicators.
