Community health information platform (CHIP) is a unified comprehensive information management and information exchange system, as the core files to the health of residents, community-based health care information. It serves the region's community health agencies, medical institutions and public health authorities. In CHIP, the different business of the regional community health agencies, medical institutions and public health authorities was analyzed, classified and specified. We solved the exchange of information and sharing of health problems between the community residents, general practitioners, health and medical institutions and public health authorities and achieved regional health information.According to the national requirements for the City and County of CHIP, we studied the status of the domestic and international community health services. We also investigated the problems existing in the construction and development of the CHIP at all levels. We proposed the general idea to build the CHIP as the core of the health records.We analyzed the various business functions of users with the CHIP and non-functional requirements according to the health information exchange among the community residents, the general practitioners, the community heath agencies, the all-around medical institutions, and the health administration.And thus we designed a case diagrams of the CHIP for users, sequence diagram and data flow diagram. We also described the software system requirements.According to the functional requirements for various users, we developed and designed the framework of the CHIP to obtain access to health records of3 level-2 modules and 7 level-3 sub-modules. Considering the characteristic of a paperless work with electronic health records, we analyzed the various types of user data exchange and access to services. We also designed in detail the overall system functionality for the platform and platform capabilities of the subsystems and the user login process. And thus the platform can meet the needs of the users.To various types of data exchange and access to the user and in the principles of the unified centralized management and storage of data, we designed a database system of the CHIP. We were directed against the business and functional requirements on the database performance and optimized them. The design basically meet the needs of the community health information data exchange in the City and County area and information sharing needs.We designed a security system in the CHIP from physical security, network security, application security and management of security point of view. And then we proposed the scheme to ensure the safe operation of the CHIP through the techniques and means of monitoring system in the engine room, recovery from data disaster, network firewall, virus prevention and control, unified authentication, and specifications in safety management. Finally, we carry out the designing of community health information platform.To ensure the quality of design and software, we have tested the health records management and information sharing health records for the main function of CHIP.The test results show that the design is correct, feasible and effective.
