Baekground and Purpose:Vibration Response imaging System (Vibration Response Imagine System, VRIxp) is Israel's Deep Breeze company research and development production patent products, is one of important invention at the beginning of this century, since the System appeared in the market, in breathing epidemiology fields, supplying a dynamic and objective basis for the respiratory disease diagnosis and monitoring, so as to open a new path in this field.Vibration Response Imaging (Vibration Response Imaging, VRI) diagnosis system as a new type of lung inspection method is gradually widely used in clinical, the system transfer the lungs sound signal into dynamic image, and presented on a display, in a sense, this system is one continuation of the traditional stethoscope, but it is better than traditional stethoscope is that where it can get the position of the subjective information into the objective data, and can permanently store down and repeat playback, so as to improve the lung disease diagnosis and monitoring level, to make up for the lack of existing check to a certain extent.This experiment will research the difference of VRI between Chronic Obstructive Pulmonary disease (Chronic Obstructive Pulmonary Diseases, COPD) and healthy controls, find out the diagnosis through this system for Chronic Obstructive Pulmonary disease, and research the hierarchical value of this system for Chronic Obstructive Pulmonary disease, meanwhile, research the clinical symptoms, lung function and VRI change of Chronic Obstructive Pulmonary disease patients before and after treatment, discusses whether VRI for Chronic Obstructive Pulmonary disease in curative effect with a new way.Materialsandmethods:Select153cases of Chronic obstructive pulmonary disease suffered acute exacerbation (Acute exacrbations of Chronic obstructive pulmonary diseases, AECOPD),the patients were hospitalized respiratory medicine department of Kunming general hospital from August2010to February2011. Among the COPD patiens,129patients were male and24patients were female. average age were (66.78±10.66) years old.COPD group meets the diagnostic criteria in COPD Diagnosis and Treatment Guidelines (2007):lung funetion FEV1/FVC<70%(Forced expiratory volume in one seeond/forced vital capacity), age between13-82years old;recently either used bronchodilators or were washed off.Healthy volunteers were57(control group),among the volunteers.Including28male and29female, with the average age were (48.82±17.07) years old.Into the standard of Healthy volunteers were:lung function FEV1/FVC>75%%and FEV1%>80%, FVC%>80%, the exclusion standard of experimental group and control group were:thoracic spinal deformity; Back skin damage; Lung cancer or bronchial asthma.For all the chronic obstructive pulmonary disease group of patients and healthy volunteers were performed general physical examination, traditional lung function testing, VRI and related inspection, Through the approach of spasmolysis and smoothing asthma treatment,wait for chronic obstructive pulmonary disease groups of patients, symptoms remissed, general condition get better,than detecting again through examinations above.Before treatment, observating the difference in VRI images and related parameters (vibration energy curve characteristics, the biggest energy frame, dynamic energy images, dry or wet Rales, lung quantitative data) between the group of patients with chronic obstructive pulmonary disease and healthy volunteers, and comparing with traditional lung function, discussing classified value of the VRIxp.meanwhile, observing VRI images and related parameters change before and after treatment in patients with chronic obstructive pulmonary disease.With SPSS17.0statistical analysis software package for data processing, measurement data was expressde in χ±s, groups comparison was tested by the grouping t-test and matching t-test, rates comparison was tested by x2,the correlation analysis of the two samples was spearman correlation analysis,P<0.05was considered statistically significant.Results:1.The VRI curve of the healthy volunteers is smooth, have the peak of inspiration and experation,with few abnormal performance, amplitude is from1.5to3.5, no jumping in the image, no dry or wet rales.and the amplitude of COPD patients is a bit low, the abnormal performance of the curve is more wide,compared with healthy volunteers,the amplitude and ratings of the curve have significant difference (P>0.05);2.Patients with COPD severity classification is through the determination of the FEV1%, in patients with COPD,selecting amplitude and curve score of VRI resuls for spearman correlation analysis, the r value were0.583,0.498, a moderate related.3.After COPD Patients were hospitalized,with spasmolysis and relieving asthma,the value of FEV1was improved since before (P>0.05),the VRI amplitude and curve score compared treatment before were significant difference (P>0.05).Conclusion:1.The VRI results of Chronic obstructive pulmonary disease (COPD) patients and healthy volunteers were in a significant difference that can help COPD diagnosis.2.VRI results and chronic obstructive pulmonary disease (COPD) classification has a good correlation, among them, both of the bouncing feeling and curve score, have a moderate related with chronic obstructive pulmonary disease (COPD). 3.Before and after treatment of chronic obstructive pulmonary disease (COPD) patients, with the improvement of the FEV1, amplitude and curve scores in VRI have significant differences,this can be thought of as VRI in judging COPD treatment effection have a good bjectivity.
