Objective: To explore the value of MSCT for assessing the lung volume, analyze the relationship between LAA% and PFT indexes, LAA% and BODE score, to explore the application value of CT lung volume measurement in stable chronic obstructive pulmonary diseases, including the value of diagnosis and comprehensive assessment.Methods: A stratified cluster random sampling method, Changzhou City Clock Tower District Yonghong, resistant permanent residents over 40 years 4926 cases for the survey, design standards epidemiological questionnaire in December 2014 to March 2015 for sampling objects investigation. Among them,190 patients with stable COPD and 143 healthy people were performed the examinations of the lung function,mMRC score,CAT scale,6MWT and MSCT examination. The difference of PFT indexes and LAA% in case group(A,B,C,D groups) and the control group comprehensively evaluated, as well as the correlation among the LAA%,PFT indexes and BODE scores. All data analyzed with SPSS 19.0, P<0.05 was shown for outcome.Results: 1. PFT indexes:The FEV1% predã€FVCã€MMF ã€PEFã€PEF25ã€PEF50ã€PEF75 were statistically significant difference respectively in control, A,B,C,D five groups.(P<0.01), for MMF and PEF,C and D groups respectively were less than A and B and control groups, the differences were statistically significant(P<0.05).A and B and control, C and D between-group differences were not statistically significant(P>0.05).But for FEV1%predã€FVCã€PEF25ã€PEF50ã€PEF75, C and D groups respectively were less than A and B groups, and they were less than control groups, the differences were statistically significant(P<0.05). A and B,C and D between-group differences were not statistically significant(P>0.05).2.MSCT: The LAA% was statistically significant difference respectively in control, A,B,C,D five groups.(P<0.05),and the control group was less than group A, than they were less than B,C groups,and also all the four groups were less than group D, the differences were statistically significant(P<0.05). B and C between-group difference was not statistically significant(P>0.05).3.Correlation:(1) The LAA% and each PFT indexes like FEV1%predã€FVCã€MMFã€PEFã€PEF25ã€PEF50ã€FEF75 all has significant negative correlation(all P<0.01),and in each group they have no correlation(all P>0.05).(2).The LAA% and BODE score have significant negative correlation(P<0.01), and in group A and B, they have no correlation(all P>0.05), but in group C and D, they have significant negative correlation(P<0.01).Conclusions: MSCT could rapidly and accurately assess LAA%, it was correlated each PFT indexes like FEV1%predã€FVCã€MMFã€PEFã€PEF25ã€PEF50ã€FEF75 and the BODE scores, so we can find that LAA% has some clinical value for early diagnosis of COPD patients, and can reflect the degree of airflow restricted severity, small airway function, airway patency and early, middle and late stages of gas flow, as well as the general situation of COPD patients. |
