| Objective: By correlating the inflammatory markers neutral lymphatic ratio(NLR),platelet lymphatic ratio(PLR)and urinary osteopontin(OPN)in patients with Chronic Obstructive Pulmonary Disease Complicated with Lung Cancer COPD-LC and Chronic Obstructive Pulmonary Disease(COPD)By correlating the inflammatory markers neutral lymphatic ratio(NLR),platelet lymphatic ratio(PLR)and urinary osteopontin(OPN)in COPD patients,we attempted to construct a screening system for COPD-LC high-risk groups and analyze the relationship between NLR,PLR and patients’ clinical manifestations to improve the understanding of NLR and PLR.Methods: By statistically analyzing the clinical data of 35 COPD-LC patients and 53 COPD patients hospitalized in the Fourth Clinical Medical College of Xinjiang Medical University from August 2021 to March 2023,we divided the according patients into COPD-LC group and COPD group, and analyzed the data of clinical test results, pulmonary function and TCM symptom score of the two study groups by statistical methods, respectively. Results: The area under the ROC curves of NLR,PLR and OPN protein were 0.749,0.751 and 0.915,respectively,and their sensitivities were 60.0%,57.14% and 88.57,and specificities were 84.91%,86.79% and 66.04%,respectively,in the CL group. The sensitivity was 60.0%,57.14%,88.57,and the specificity was 84.91%,86.79%,66.04%,respectively;when the three were combined, the sensitivity was 85.71 and the specificity was 81.73,respectively.2.The decrease in FEV1% predicted value was negatively correlated with NLR(r=-0.250,p=0.003)and PLR(r=0.237,p=0.005),and the correlation was higher than other inflammatory indexes;3.The results of the correlation analysis of TCM syndrome score showed that the TCM syndrome score was positively correlated with NLR(r=0.316,p<0.001),PLR(r=0.305,p<0.001)and OPN protein(r=0.331,p<0.001);5.In the TCM organ deficiency classification group, patients with lung, spleen and kidney deficiency had higher NLR and PLR than those with lung and kidney deficiency.NLR and PLR were higher than those patients with deficiency of both lung and kidney and deficiency of both lung and spleen; Conclusion: NLR and PLR urinary OPN protein were significantly higher in COPD-LC patients than in COPD patients.2.NLR and PLR showed a negative correlation with the expected value of FEV1% in patients.3.TCM symptom score was positively correlated with NLR,PLR and OPN protein.4.The NLR and PLR were higher in patients with deficiency of lung, spleen and kidney than in patients with deficiency of lung and kidney and deficiency of lung and spleen in the group of organ deficiency. |
