Objective:(1) To compare the incidence of anxiety and depression between inflammatory bowel disease and irritable bowel syndrome.(2) To study the relevant factors of anxiety and depression in patients with IBD and IBS, in order to provide theoretical basis of the anti-anxiety depression and psychological treatments for IBD and IBS patients with psychological abnormalities.Method:60patients with IBD from December2010to December2011were collected in Department of Gastroenterology in the First Affiliated Hospital of Kunming Medical University. During the same period,60patients with IBS and60healthy controls were collected.Patients with IBD, IBS and healthy controls were assessed by general questionnaire, Zung self-rating anxiety scale, Zung self-rating depression scale, life event scale and social support rating scale.All scores were compared by SPSS20.0statistics, P<0.05was considered to have statistic difference, P<0.01was considered to have significant statistic difference.Result:(1) Comparison of incidence of anxiety and depression:the incidence of anxiety was IBS group>IBD group>healthy controls, pairwise comparisons among three groups was P<0.05. The incidence of depression in IBD and IBS groups was higher than healthy controls (P<0.01). The incidence of depression between IBD and IBS had no difference (P>0.05).(2) Results of self-rating anxiety scale, self-rating depression scale score showed that there was no statistic difference between IBD group and IBS group (P>0.05). However, the scores of two groups of patients were higher than healthy controls(P<0.01).(3) Life event scale, social support rating scale results showed that:there was no significant difference in positive events scores and social support total scores (P>0.05).there was significant statistical difference in negative events scores (P〈0.01) between IBD groups and healthy controls repectively. IBD group compared with IBS group showed that there was not statistic difference in negative events scores (P>0.05).(4) Comparisons of anxiety and depression correlation factors:there was remarkable statistics differenc in age and negative events scores (P〈0.01), IBD and IBS groups had a common risk factor of producing anxiety which was negative events scores, OR value was1.041and1.047respectively; Gender, age, negative events scores were risk factors of anxiety scores (P<0.01). negative events scores were the common risk factor between IBD and IBS group in depression, OR value respectively was1.079and1.125. Gender, age, negative events scores were risk factors of depression scores (P<0.01).Conclusion:(1) Incidence of anxiety and depression in IBD and IBS patients is higher than the normal.(2) Negative life events have an important mentally impact on IBD and IBS patients.(3) life events and social support are closely related to psychological changes of IBD and IBS patients.(4) IBD and IBS patient's psychological state should be concerned in clinical practice and appropriate interventions should be given if there is a clear anxiety and depression in the patients.
