ObjectiveStandardized training of resident physicians first aid skills will help for enhancing the ability of first-aid practices , improving the hospital's success rate, and improving the quality of first aid training standardization. At present, a number of large general hospitals are focusing on first-aid medical skills training, regularly training and assessing first aid skills for all doctors, and achieved good results. However, in the primary hospital, due to limited conditions, it is difficult to perform effective training for hospital emergency medical skills. Because there is no uniform standard, most of the regional central hospital train and assess for residency in their own way. There are some problems during the process of training, especially emphasis on teaching the theory was more than first aid skills practice. Thus, it has an important significance to investigate the effective methods of standard training for first aid skills in regional central hospital.This study aimed to find an effective method fitting regional hospital center for standard first aid skills training, and gradually promote in primary hospitals.Methodsthe content of training is refer to "the standardization of clinical residency training programs" and "practitioners clinical skills exam outline" of relevant contents, combined with the hospital's residency training experience. Based on 10 item of residency first aid skills training in 2009 and 2010 in our hospital, 60 residents from different regional center hospital in Chongqing municipal were trained by computer simulation teaching focused on skill-based and traditional face to face methods , assessed under OSCE (Objective structure clinical examination) uniform standard in 2011, and assessed by uniform standards. At the same time, questionnaires which survey the training requirements and degree of knowledge of residents were issued, and the training current situation of all of central regional hospital were investigated.statistical method All information data entried by SPSS13.0 statistical software, Descriptive statistics were used by frequency, composition ratio, the mean and standard deviation. Chi-square test was used for general comparisons, using two independent sample rank sum test was used for the performance comparison. Part of the questionnaire content using Excel spreadsheet for statistical analysis.ResultsExamination results: in computer simulation teaching focused on skill-based group, ECG examination average score was73.67±16.001; cardiopulmonary resuscitation score was 80.69±5.809, endotracheal intubation score was 95.10±3.067, Executive Medical score was 385.60±19.174; while in traditional way group, the scores were 53.20±15.473, 77.00±8.077, 83.83±10.198, 368.57±24.749, respectively. The score and effectiveness of computer simulation teaching focused on skill-based methods were significantly better than traditional face to face training methods (P <0.05).Hospital Survival rate Computer simulation teaching focused on skill-based training was performed in our Hospital in 2009, the hospital survival rate of 77.28%. , the effect of training emerged gradually in 2010, the hospital survival rate increased to 86.79%. Through statistical analysis of these results shows that training with computer simulation teaching focused on skill-based and assessment under OSCE was superior to the traditional face to face manner and was a good methods of future training in regional central hospital.Questionnare survey general conditions of our hospital and three other regional centers hospital doctors were not statistically different (P> 0.05); understandings of training in all of hospitals were similar: 80% of the residents participated the regulation training, and were very much needed standard and more systematic first aid training; the main source of first aid skills were clinical practice and simulation training. implementation and evaluation aspects: all of hospitals were not enough emphasis on first aid training; teaching methods are simple, mainly to face to face; the pressure of examination and assessment were small.; training and impact factors: 61.7% of residents believed that first-aid skills training could improve the clinical skills of first aid and had a important role for clinical work. 83.3% of trainee did not receive a certificate after training ConclusionsStandardized resident training of first aid skills with computer simulation teaching focused on skill-based and assessment under OSCE uniform standards can improve their first aid skills and hospital survival rate, and enhance the quality of standardized training, save the lives of critically patients maximatily, is an effective training methods and assessment approach for regional central hospital residents standardized training of first aid skills.
