Objective:1.To investigate the incidence of vascular cognitive impairment (VCI) in inpatients with Hypertension of different stage.2. To find the independent risk factors and associated risk factors for VCI in inpatients with Hypertension of different stage.Methods:Select 220 hospitalized patients diagnosed hypertension from Department of Neurology and Cardiology in the North hospital of Second Affiliated Hospital of Dalian Medical University, from September 2009 to December 2010 . Patients were divided into 2 groups of 68 patients with hypertension stage2 and 152 cases with hypertension stage 3.Every patient was assessed by the Montreal Cognitive Assessment(MoCA),and investigated about the basic information(age,gender,educa- tion.ect), and if he had the factors which included with or without the history of Smoking,Diabetes, Coronary Heart Disease, Atria Fibrillation,TIAs,Carotid Atheros- clerosis,Cerebral Infraction Hypercholesterolemia,ect. According to the standard classification of Hypertension and the MoCA result, the entire sample was been divided into two teams and 4 small groups for the further researching. Team Hypertension Staged2(THS2), Team Hypertension Stage3(THS3)and THS2-VCI(+),THS2-VCI(-),THS3-VCI(+),THS3-VCI(-).1.Investigating the incidences of VCI in the inpatients with hypertension stage 2 and hypertension stage 3 and finding if there were statistical differences between them.2. According to the scores of MoCA,each team were divided into one group with VCI and the other group without VCI. To find the differences of the factors between two groups respectively in each team. 3. Finding out the difference of the scores of MoCA between THS2-VCI(+) and THS3-VCI(+) .4. List differences of severity of factors in the analysis and take them as indepen- dent variables, VCI as dependent variable for logistic diverse regression analyzes. To find the independent risk factors for VCI in both groups.Results:1. The results show the incidence of VCI in the inpatients with hypertension stage2 is 44.12%, in the the inpatients with hypertension stage3 is 61.18%, There is a statistical difference between them, P<0.05.2. Univarinate- Analysis:1) In the team Hypertension stage2,for the Age factor, VCI(+) group is obviously elder than the VCI(-) group, but the education condition factor is showing reversal. Cerebral Infraction ,Atria Fibrillation and Carotid Atherosclerosis also have great increasing than the VCI(-) group,P<0.01 or P<0.05;2) In the team Hypertension stage 3,the age of VCI(+)group is obviously elder than the VCI(-)group too,the education condition factor is showing reversal. Smoking, Cerebral Infraction ,Diabetes and Carotid Atherosclerosis have great increasing compare with VCI(-) group. P<0.01 or P<0.05.3) According to the compared results,There is no significant difference from Mon- treal Cognitive Assessment of VCI(+) between the Team Hypertension Stage2 and Team Hypertension Stage 3, P>0.05.3. Non- Condition Logistic Regression Analyzes: The independent risk factors of Hypertension stage2 for VCI contains Age ,and Cerebral Infraction. For Hypertension stage 3,Age ,Education Condition and Cerebral Infraction are the independent risk factors for VCI.Conclusion:1.The incidence of VCI is 44.12% in the inpatients with hypertension stage2 and 61.18% in the inpatients with hypertension stage3, there is a statistical difference between the two teams.2. Advanced Age, Low Education, Cerebral Infraction, Atria Fibrillation and Carotid Atherosclerosis are the risk factors of VCI in the inpatients with hypertension stage2.The High Age and Cerebral Infraction are the independent risk factors for VCI in the inpatients with hypertension stage2.3. The risk factors of VCI in the inpatients with hypertension stage3 includes Advanced Age, Low Education, Diabetes, Cerebral Infraction ,Carotid Atherosclerosis and Smoking. Among which, Advanced Age, Low Education, Diabetes and Cerebral Infraction are the independent risk factors for VCI in the inpatients with hypertension stage3.4. There is no significant difference of severity for VCI between Hypertension groups of stage2 and stage3.
