Abstract/Summary:
Medical and health services are closely related to the vital interests of the common people. The issue of "difficult and high cost to access medical service" and other issues are affecting the social stability and harmony. Medical and health system, therefore receives a high degree of attention by the whole society. Among them, the reform in public hospitals is mostly concerned to be the hot and difficult issues, and the compensation mechanism is the focus of the reform in public hospitals.This research mainly studies on the pilot reform of the compensation mechanism conducted in Luodian Hospital in Baoshan District. This research analyses and evaluates the pilot reform which aims to standardize the revenue and expenditure management in hospitals, to improve the quality of medical care, to reduce medical costs, to increase the government investment in hospitals, and to balance the revenue and expenditure management. This research also aims to give a true and objective reflection of the effects of the reform, to continuously improve the compensation mechanism in public hospitals, to serve as a basis to optimize and to further promote the reform program, and to provide a useful reference to further implementation of the compensation mechanism reform in the public hospitals.This study collected a large number of financial statements and statistical data, conducted surveys in the method of questionaire, accurately summarized the relevant data, and performed detailed data analysis to find the problems and their causes during the reform process. The study also targeted to make specific recommendations under the requirements of the new medical reform.An obvious overall effect was noted after the compensation mechanism reform was carried out in Luodian Hospital. It generated a good effect of motivating the medical staff, enhancing the stability of the medical team, reducing medical costs, improving quality of care, and increasing the governmental financial compensation. Meanwhile, the study also noted that certain issues need to be further addressed, such as security of financial resources, effective monitoring of public hospitals, public hospitals' own internal operation and management.This study suggests that it requires the joint efforts from the government, health authorities, public hospitals themselves and all sectors of society to further implement the compensation mechanism reform in the following aspects:1) it is necessary to further strengthen the government's leading role in the reform of public hospitals by implementing their funds invested in health laws and regulations, and system security; 2) it is necessary to strengthen the health administrative departments'supervision of public hospitals, to accelerate the reform of the "Separation of Hospital Management from Hospital Operation" system, and to enhance the effectiveness of supervision; 3) it is necessary to improve the operation system within the hospital, to improve their management proficiency, and to meet the requirements for reform; 4) it is necessary to develop regional science and health planning, to coordinate medical resources, and to improve the efficiency of financial investment; 5) it is necessary to accelerate the establishment of price system which reflects the value of the medical staff services. This study provides certain level of value to be referred to in theory and practice. However, due to the author's limited knowledge, there is still a long way to go to further understand the policy of compensation mechanism in public hospitals and the research on its theoretical system.
