| Background:Acute myocardial infarction (AMI) is a common and frequently-occurring disease, which influence human health and quality of life seriously.It happens urgently, develops rapidly and has a high mortality rate. With the changes of life style, diet structure and social environment, it has been found that the incidence of AMI and mortality trends were increasing year by year. While research for AMI by traditional Chinese medicine (TCM) is developing, most physician have make a consensus that the pathogenesis of AMI is both Qi-deficiency and blood stasis simultaneously with wet turbidity. And the major syndrome of AMI is fundamental deficiency and severe external symptoms. Therefore, the treatment of nourishing Qi,activating blood circulation to dissipate blood stasis, clearing out wet turbidity becomes more important in the therapy of TCM on AMI. At the same time, integrative treatment by differentiation of symdromes (TDS) and diseases can improve clinical curative effect.Yugengtongyu Decoction is founded by academician Keji Chen according to his decades of clinical experience. Consider to the pathogentic characteristics of AMI, it is an effective decoction with the therapy of nourishing Qi, activating blood circulation to dissipate blood stasis, clearing out wet turbidity. This study is emphasis on the shot-turm prognosis and quality of life influented by Yugengtongyu Decoction on AMI patients, who had been treated by routine western medicine or thrombolysis, PCI. We also focus on the difference of TDS and treatment by differentiation of diseases (TDD)Objective:Through the collection of the clinical data and questionnaire survey of AMI patients, to find out the influence of the prognosis and quality of life on AMI patients who have taken Yugengtongyu Decoction base on treatment by routine western medicine or thrombolysis, PCI.Then to lay the foundation for further exploration of the prevention and treatment of AMI by integrative traditional Chinese and Western medicine, to contribute to prescription on inheriting and carrying forward and promoting the advantages of the old Chinese medicine practitioners, and promoting the theory of Chinese medicine theory.Method:The study population consisted of 97 AMI patients in XiYuan hospital from January,2009 to October,2010. The patients were divided into 3 groups by whether taking Yugengtongyu Decoction or not. The case history of all the patients were read and the prognosis outcomes of each patient were known by interview, including main end-point, secondary end-point and quality of life. To compare the differences of the Indicators above after 6 months. Measurement data was represented by mean±standard deviation. T test and analysis of variance was used. Count data was analyzed by using chi-square test. Level information was analyzed by rank sum test.Result:The marks of Seattle angina questionnaire (SAQ) show that:the marks on physical limitation (PL),Anginal frequency (AF),Treatment satisfaction (TS),Disease perception (DP) of Yugengtongyu Decoction group was higher than the group treated by routine western medicine only. The incidence of stroke and hospitalization rate coursed by angina, arrhythmia or heart failure of the treatment group was lower than the group treated by routine western medicine only. There is no difference in the the incident of end-point and quality of life between the Yugengtongyu Decoction group and the group had taken other Decoctions.Conclusion:1. Yugengtongyu Decoction can reduce the incidence of stroke and hospitalization rate coursed by angina, arrhythmia or heart failure.2. Yugengtongyu Decoction can improve the short-turm(6 months) quality of life, andThe effects is better than the group taken westen mendicine only.3. There has similar effect between the Yugengtongyu Decoction group and the group had taken other Decoctions.It indicates the effectiveness and rationality of TDD.4. The treatment based on integrative traditional Chinese and Western medicine for AMI, can shorten the hospitalization rate and reduce hospital costs and bring economical advantages. |
