Objective:This study explored the related factors of complications during the treatment of gallstone attacked by General Offensive Paishi. To provide a scientific theoretical basis and practical guidance for the effective prevention of complications and better development of General Offensive Paishi.Methods:The thesis selected 448 gallstone patients who underwent the same treatment of gallstone attacked by General Offensive Paishi Therapy in TCM combined with Western Medicine GanDan department of Hebei united university kailuan hospital from September 2008 to June 2010 as the research objects. Among them. there were 57 cases with complications as the complication group. Meanwhile numbered the 391 patients who not appear complications based on the sequence of admission time, and selected the 195 cases for even number as the without complication group. Through accessing the related datas of records and collecting selected cases general data, relevant information of gallbladder dopplar ultrasound imaging and history data of the patients to completed questionnaires.The related factors were investigated in this study consisting of age, BMI, nationality, occupation, the volume of gallbladder, the thickness of gallbladder wall, the double-wall signs of gallbladder wall, gallbladder wall roughness, gallbladder whether or not with fold, the situation of bile silting-up, the number of gallbladder, the diameter of maximum gallbladder, the days of paishi, the history of gallstone, the history of Coronary heart disease, hypertension, dyslipidemia, chronic liver disease, pancreatitis, gastroduodenal diseases, diabetes, duodenal diverticula, gallstones family history, smoking history and drinking history. All selected cases data were integrity. Through the statistical analysis, to discuss the related factors of complications during the treatment of gallstone attacked by General Offensive Paishi therapy.The data were created database with Excel 2003 of the computer, and analysed with software SPSS13.0. Measurement data with mean±standard deviation to express, the comparison of the sample mean differences between the two groups using t-test, counting data using Chi-square test. In accordance with the inspection standards (p<0.05) the significant factors using many factors Logistic regression analysis. P<0.05 was taken as significant differences.Results:By chi-square test single factor analysis showed that drinking historiy (p=0.049, OR value=3.706), the thickness of gallbladder wall (p=0.017, OR value=2.075), double-wall sign (p=0.001, OR value=3.545), gallbladder wall roughness (p=0.000, OR value=3.562), bile silting-up (p=0.003, OR value =2.918), the number of gallbladder (p=0.025, OR value=2.468), the diameter of maximum gallbladder (p=0.019, OR value=0.493), the days of paishi (p=0.004, OR value=1.340) and the history of pancreatitis (p=0.034, OR value=7.283) had influence on complications during the treatment of gallstone attacked by General Offensive Paishi Therapy.1 Compared between the two groups of drinking history, the patients of complication group who had a drinking history were more than without complication group. There was a significant difference between the two group (p<0.05).2 Compared between the two groups of the thickness of gallbladder wall, the patients of complication group whose gallbladder wall's thickness more than 0.4 cm were more than without complication group. There was a significant difference between the two group (p<0.05).3 Compared between the two groups of double wall sign, the patients of complication group who appear double wall sign were more than without complication group. There was a significant difference between the two group (p<0.05).4 Compared between the two groups of gallbladder wall roughness, the patients of complication group with obvious rough gallbladder wall were more than without complication group. There was a significant difference between the two group (p<0.05).5 Compared between the two groups of bile silting-up, the patients of complication group who appeared bile silting-up were more than without complication group. There was a significant difference between the two group (p<0.05).6 Compared between the two groups of gallbladder'number, the patients of complication group whose gallbladder'number more than 5 were more than without complication group. There was a significant difference between the two group (p<0.05).7 Compared between the two groups of the diameter of maximum gallbladder, the patients of complication group whose maximum gallbladder's diameter less than 0.5 cm were more than without complication group. There was a significant difference between the two group (p<0.05) .8 Compared between the two groups of paishi days, the patients of complication group whose paishi days beyond 28 days were more than without complication group. There was a significant difference between the two group (p<0.05).9 Compared between the two groups of the history of pancreatitis, the patients of complication group who had suffered from pancreatitis were more than without complication group. There was a significant difference between the two group (p<0.05). There were nine factors which there a significant difference in the single factor analysis, Which were taken into many factors Logistic regression analysis, the results showed that double-wall sign, obvious rough gallbladder wall, bile silting-up, the number of gallbladder > 5 and the diameter of maximum gallbladder≤0.5 cm are independent risk factors for the patients with gallstones to appear complications during the treatment of General Offensive Paishi Therapy. OR value are respectively 2.649, 4.046, 2.751, 2.741, 0.359.Conclusion:1 Drinking historiy,the thickness of gallbladder wall, double-wall sign, gallbladder wall roughness, bile silting-up, the number of gallbladder,the diameter of maximum gallbladder, the days of paishi and the history of pancreatitis have related with complications during gallstone attacked by General Offensive Paishi therapy.2 Gallbladder double-wall sign, obvious rough gallbladder wall, bile silting-up, the number of gallbladder>5 and the diameter of maximum gallbladder≤0.5cm are independent risk factors for the patients with gallstones to appear complications during the treatment of General Offensive Paishi Therapy.
