One-stage Bilateral Total Hip Replacement: A Retrospective Study

Objective To evaluation the superiority of one-stage bilateral total hip replacement , and to discuss the indications and surgical skills of one-stage bilateral total hip replacement.Methods From January 2005 to April 2010 , 33 patients underwent bilateral total hip arthroplasty (THAs) in a single procedure , of the 33 patients , 20 were male and 13 were female , Their ages ranged from 19~67 years , mean 44 years . Their courses of disease ranged from 0.5 to 40 years , mean 8 years .Results All the 33 patients were followed up for 7 to 35 months (33 on average) . All patients were done with posterolateral approach and non-cementless prosthesis . The operative time was 90 minutes to 379 minutes , 251 minutes on average . The Intraoperative blood loss was 500ml to 4000ml , 1161ml on average . The volume of blood transfusion was 1U to 12U ( concentrated red blood cell , CRBC ) , 4.5U on average . The mean hospital stay was 23 days , ranged from 14 to 72 days . The Harris scores of the joint function after the operation ranged from 73 points to 96 points , 89.2±4.9 points on average , The good rate was 93.9% . The preoperative total range of hip motion ranged from 0°to 140°,81.2°on average . The postoperative total range of hip motion ranged from 170°to 215°,195.1°on average . Patients with ankylosing spondylitis improved most in total range of motion . During the follow-up period , 1 patient dislocation , 2 patients need unilateral crutch for their hip pain and wound infection , 3 patients still taking painkillers because of their knee degeneration .Conclusions Bilateral total hip replacement as one-single stage have many superiority , such as short hospital stay , gain the optimal functional recovery , short recovery time and save costs . We think bilateral single-stage total hip arthroplasties have an acceptable risk for patients with end-stage bilateral hip disease , and should have the priority to choose this surgical methods . Adequate preoperative preparation and the necessary surgical skills are the key to success...
