| Objective; With the knowledge and technology of oral epidemiology,medical sociology and medical statistics, we are aiming at developing an oral Health-related Quality of life rating scale suitable for China's denture restoration patients, which can provide a new method and evidence for the assessment of patients'oral health status and denture restoration treatment effect, through exploratory study of oral Health-related Qualitv of life assessment system. The specifics include:(1) The establishment of the oral Health-related Quality of life (DPORQOL) and the assessment of its reliability, validity and reaction degree;(2) Based on the DPOHRQOL. assessing its psychometric quality, exploring the factors which influence oral health-related quality of life of denture restoration patients and providing evidence for improving the design of denture restoration.Methods; (1) According to the principles and methods of International Quality of Life Assessment Project (IQOLA) and referring to the related foreign papers on oral health-related quality of life research, the foreign related rating scales were translated, backtracked, culture accommodated and refined. The rating scale items were collected through open questionnaire survey and interview, and the significances among dentists and dental patients were evaluated based on the options of the items. Then the highest scoring items which are the main factors effecting denture restoration patients' life quality were screened out to form the elementary rating scale item pool of DPOHROOL.(2) 61 patients who are meeting inclusion and exclusion standards in prosthodontic department of stomatology school of The Forth Military Medical University (FMMU) from 2010.3.9 to 2010.4.25 were surveyed about their oral health-related quality of life with the elementary rating scale item pool. The patients finished the questionnaires including informed consent, demographic information and DPOHRQOL items. The items with the minimum factor load coefficient were deleted according to the factor analysis of the results. After analyzing and screening, we developed the oral Health-related Quality of life elementary rating scale on denture restoration patients.(3) 286 patients who are meeting inclusion and exclusion standards in prosthodontic department of stomatology school of The Forth Military Medical University (FMMU) from 2010.6.5 to 2010.8.20 were surveyed with statistical methods such as variation coefficient, correlation coefficient and factor analysis. The items were filtrated based on importance, sensibility and representation. The difference of rating scale scoring on demographic factor and the effect factors of oral health-related quality of life were analyzed, and the formal DPOHRQOL was established.(4) 104 denture restoration patients were surveyed about the oral health-related quality of life with DPOHRQOL. The psychometric quality, reliability, validity and reaction degree of the DPOHRQOL was assessed. The differences of DPOHRQOL scoring on demographic factor and the effect factors of oral health-related quality of life were analyzed.Results; 1. The establishment of the DPOHRQOL(1) The new DPOHRQOL includes 21 items of 4 fields(aching discomfort, physiological function, psychological function and social function) (2) The DPOHRQOL internal consistency reliability (Cronbach's alpha) is 0.91, the Cronbach's alpha in all fields is greater than 0.70, the half-split reliability is 0.82, the correlation coefficient of items and scale is between 0.14-0.68;(3) Each item in DPOHRQOL has strong correlation with respective areas and weak correlation with other areas; the total score of the rating scale has significant positive correlation with self-assessment oral health and teeth number, the correlation coefficients are 0.516 and 0.649 respectivelv(p<0.01);(4) 7 common factors were selected through factor analysis, and the cumulative percent is 64.951%. The information that the selected 7 common factors reflect is in accord with the anticipatory scale structure. Comprehensive factor variables reflect the correlation of aching discomfort, physiological function, psychological function and social function;(5) The correlation coefficients of DPOHRQOL scoring and self-assess oral health, treatment needs are -0.456 and -0.277 respectively (p<0.01). It is moderate related with OH-QOL total score, the correlation coefficient is -0.654(p<0.01);(6) DPOHRQOL can obviously distinguish oral health condition of different communities, the score has significant difference between oral healthy group and oral unhealthy group(p<0.01);2. The application of DPOHROOL(1)There is no difference between sex, age and education degree but family economy about oral heath-related quality of life;(2)The score of oral heath-related quality of life of denture restoration patients is positive correlated with the lost teeth number;(3)There is no internal relations between "oral ulcers or gingival bleeding symptom" and different restoration methods.(4)The oral health-related quality of life of fixed partial denture group is better than removable partial denture group and complete denture group(5) Physiological function has more effect on oral health than other three fields.Results; This studv successfullv developed DPOHRQOL and proved its good reliability. validity and reaction degree. DPOHRQOL has the potential to become a new method of assessing the oral health status and oral health-related qualitv of life of denture restoration patients in China. Through the application of DPOHRQOL. it is found that different denture restoration methods have obvious effect on oral health-related quality of life, especially on physiology function. |
